A¤r›l› Ayak Bile¤inde Heterotopik Ossifikasyon: Olgu Sunumu<br />

Heterotopik Ossifikasyon (HO) kas ve ba¤ dokusunda oluflan ekstraartiküler yerleflimli patolojik<br />
yeni kemik oluflumudur. S›kl›kla kafa travmas›, omurilik yaralanmalar›, serebrovasküler<br />
olaylar, k›r›klar ve ç›k›klar ve eklem replasmanlar› sonras› görülen komplikasyondur.<br />
Patofizyolojisi karmafl›k olup, farkl› teoriler aç›klanm›flt›r. Olgumuz 37 yafl›ndaki erkek, sol<br />
ayak bile¤inde tekrarlayan burkulma ile bafllayan 4 ayd›r geçmeyen ayak bilek a¤r›s› sonras›<br />
talo-fibular ligamanda heterotopik ossifikasyondur. Öyküsünde ayak bile¤inde burkulma<br />
d›fl›nda, enfektif, metabolik, inflamatuar özellik olmayan hasta, analjeziklere k›smen yan›t<br />
veren, ancak giderek artan ayak bilekten medial malleole yay›lan a¤r› ile baflvurdu. Ayak bilek<br />
hareketlerinin aç›k a¤r›l› olmas› d›fl›nda, medial malleolde minimal ödem mevcuttu. Is› art›fl›,<br />
k›zar›kl›k, instabilite yoktu. Biyokimyasal parametrelerinde (alkalen fosfataz, sedimantasyon,<br />
CRP, RF) özellik yoktu. 2 yönlü direkt grafi do¤ald›. Çekilen ayak bilek MR sonucu anterior<br />
talofibular ligaman trasesi boyunca heterotopik ossifikasyonlar izlendi. Üç fazl› kemik sintigrafisin<br />
immatür heterotopik ossifikasyon ile uyumlu idi. Hastaya ayakkab› modifikasyonu<br />
önerildi, egzersiz ve indometazin baflland›. A¤r›lar›n›n devam etmesi üzerine elektroterapi<br />
eklendi. Ayak bilek a¤r›s› sorgulan›rken mekanik, inflamatuar, enfektif ve dejeneratif nedenler<br />
sorgulan›r ve araflt›r›l›rken heterotopik ossifikasyon da nadir görülen neden olarak ak›lda<br />
tutulmal›d›r.<br />

Miyofasiyal A¤r› Sendromunda Lokal Anestezik Enjeksiyonu ile<br />
Ekstrakorporeal fiok Dalga Tedavisi (ESWT) Etkinli¤inin Karfl›laflt›r›lmas›<br />

AMAÇ: Miyofasiyal a¤r› sendromu (MAS) olan hastalarda lokal anestezik ile tetik nokta enjeksiyonunun<br />
(TNE) etkinli¤ini, Ekstrakorporeal fiok Dalga Tedavisi (ESWT) etkinli¤i ile<br />
karfl›laflt›rmakt›r.<br />
GEREÇ-YÖNTEM: Çal›flmaya Travell ve Simons’un MAS tan› kriterlerine uygun 79 hasta (56<br />
kad›n, 23 erkek) al›nd›. Birinci gruba (n=39) lokal anestezik ile TNE, ‹kinci gruba (n=40) ise<br />
ESWT uyguland›. <strong>Hastalarda</strong> a¤r› fliddeti, VAS (vizüel analog skala) ve palpasyonla a¤r› skorlamas›<br />
ile; a¤r› efli¤i, algometre ile; yorgunluk ve fonksiyonel durum VAS ile; yaflam kalitesi,<br />
Nottingham sa¤l›k ‹ndeksi (NHI) ile ve psikolojik durum, Beck depresyon ölçe¤i (BDI) ile de-<br />
¤erlendirildi. De¤erlendirmeler tedavi öncesinde, tedaviden sonra 1.haftada ve 1.ayda toplam<br />
3 kez yap›ld›. TNE grubuna %2’lik 1 ml Prilokain, bir adet 0.45x13 mm’lik, 26 gaugelik steril insülin<br />
i¤nesi ile, i¤ne de¤iflik yönlerde yelpaze fleklinde seri olarak hareket ettirilerek, enjekte<br />
edildi. ESWT grubunda tetik noktalara 10 Hz frekansta, 1,8-3,0 bar bas›nç aral›¤›nda 400-800<br />
at›m radyal flok dalgas›, 5-8 gün ara ile toplam 3 seans olacak flekilde uyguland›. Tüm hastalara<br />
ev program› olarak germe ve postür egzersizleri verildi.<br />
BULGULAR: Her iki hasta grubunda da tedavi sonras› 1. hafta ve 1. ay de¤erlendirmelerinde<br />
VAS a¤r›, VAS fonksiyonel yetmezlik, bas›nç a¤r› efli¤i ve BDI skorlar›nda benzer flekilde anlaml›<br />
iyileflmeler görülmüfltür. ESWT grubunda, VAS a¤r› skorundaki azalma, tedavi sonras›<br />
1.ayda da ayn› oranda devam etmifltir. VAS yorgunluk skoru tedavi sonras› 1. hafta ve 1. ayda<br />
TNE grubunda ESWT grubuna göre daha fazla iyileflme göstermifltir. NHI, tedavi sonras› 1.ayda<br />
ESWT grubunda TNE grubuna göre daha fazla iyileflme göstermifltir.<br />
SONUÇ: Literatürde MAS tedavisinde daha önce bildirilmifl ESWTçal›flmas› yoktur. Bu yöntemin<br />
hastalar›n a¤r›s›n› azaltmada, fonksiyonel ve psikolojik durumunu iyilefltirmede TNE kadar<br />
etkin olup, MAS tedavisinde öncelikli bir tedavi seçene¤i olabilece¤i görüflündeyiz.<br />

Heterotopic Ossification in Painful Foot Ankle: Case Report<br />

Heterotopic ossification is a new bone formation in extraarticular locations, muscles and connective<br />
tissue. It’s a frequent complication after head trauma, spinal cord injuries, cerebrovascular<br />
diseases, fractures, dislocations and joint replacements. The pathophysiology is<br />
complicated and based on several theories. In this case; 37 year old male presented with consistent<br />
left foot ankle pain for 4 months with talo-fibular ligament ossification formation,<br />
developed after repeated several ankle injuries, There was minimal edema in medial malleol<br />
and foot ankle movements were open painful. We found no infective, metabolic, inflammatory<br />
features in the patient's history. Erythema, heath and instability were not observed. There<br />
were no pathological findings in biochemical parameters and radiographic examinations of<br />
both sides. We observed heterotopic ossification among anterior talofibular ligament in foot<br />
ankle MRI. Three phased scintigraphy showed heterotopic ossification. We advised shoe modification<br />
to the patient and took up exercise and indometasine treatment. Electrotherapy<br />
added to treatment for persistent pain. While evaluating foot ankle, mechanical, inflammatory,<br />
infective and degenerative causes can be questioned and as a rare cause heterotopik ossification<br />
can be in differential diagnosis.<br />

The Comparison of Trigger Point Injection with Local Anesthetic and<br />
ESWT (Extracorporeal Shock Wave Therapy) in Myofascial Pain Syndrome<br />

OBJECTIVE: The aim of the study is to compare trigger point injection using local<br />
anesthetic and ESWT in the treatment of Myofascial Pain Syndrome (MAS).<br />
MATERIALS-METHODS: 79 MAS patients (56 female, 23 male) who were diagnosed<br />
according to Travel and Simons’ criteria were included in the study. 39 patients were treated<br />
by trigger point injection with local anesthetic and 40 patients using ESWT. Both groups were<br />
evaluated for pain and pain threshold using VAS, pain score by palpation and algometry;<br />
fatigue and disability using VAS; life quality by Nottingham Health Index (NHI) and<br />
psychological status using Beck Depression Index (BDI). The evaluations were performed at<br />
baseline, one week after the treatments and one month after the treatment. In the 1st group,<br />
1 ml 2% prilocain was injected into the trigger point moving the needle quickly in different<br />
directions. In the 2nd group ESWT was applied to the trigger point, 400-800 hit 10 Hz<br />
frequency between 1,8-3 bar pressure range in 3 sessions with 5-8 days interval. A home<br />
exercise program was given to all patients.<br />
RESULTS: VAS pain, VAS disability, pressure pain threshold and BDI scores improved<br />
significantly in both groups at the first week and first month controls. The decrease in VAS<br />
pain scores of ESWT group continued with the same speed in first month control. NHI scores<br />
of ESWT group were better than TNE group at first month control.<br />
CONCLUSION: As far as we know, this study is the first that uses ESWT in MAS treatment. We<br />
suggest that, this technique can be as effective as trigger point injection in pain, functional<br />
and psychological status improvement parameters.<br />
