<strong>OSCA</strong> <strong>11</strong>-<strong>036</strong> Treatment Court Specialized Service Providers<br />
Federal Funds Requirement<br />
The program understands and agrees the procurement(s) may involve the expenditure of federal<br />
funds. Therefore, the program will not issue any statement, press releases, or other documents<br />
describing projects or programs funded in whole or in part with federal money unless the prior<br />
approval of <strong>OSCA</strong> is obtained.<br />
Invoicing and Payment<br />
Immediately upon award of the contract, <strong>Addiction</strong> <strong>Awareness</strong> llc shall submit or will have already<br />
submitted a properly completed State Program ACH/EFT Application, since the state of Missouri<br />
intends to make contract payments through electronic funds transfer (EFT).<br />
If not already submitted, <strong>Addiction</strong> <strong>Awareness</strong> llc shall obtain a copy of the State Program<br />
ACH/EFT Application and completion instructions from the Internet at:<br />
http://oa.mo.gov/acct/<strong>pdf</strong>files/program_input_ach_eftd.<strong>pdf</strong><br />
<strong>Addiction</strong> <strong>Awareness</strong> llc will submit a unique invoice number with each invoice submitted.<br />
The unique invoice number will be listed on the state of Missouri’s EFT addendum record<br />
to enable <strong>Addiction</strong> <strong>Awareness</strong> llc to properly apply <strong>OSCA</strong>’s payment to the invoice<br />
submitted.<br />
<strong>Addiction</strong> <strong>Awareness</strong> llc shall submit:<br />
A monthly detailed invoice to the treatment court(s) no later than the tenth (10 th ) day of<br />
the month immediately following the most recent month of service. The invoice will<br />
itemize the total number of units (quarter‐hours/hourly/daily/per test) on the invoice<br />
each month. The pricing of each unit shall be in accordance with the pricing page.<br />
A detailed monthly log of all participants and the services provided to each participant<br />
during the month.<br />
The invoices will contain all information requested by <strong>OSCA</strong>.<br />
<strong>OSCA</strong> shall pay the program in accordance with the applicable firm, fixed price stated on the<br />
Pricing Page for services actually provided.<br />
The payment shall not be based on the number of treatment court(s) participants.<br />
Other than the payments specified on the pricing page, no other payments or reimbursements<br />
shall be made to the program whatsoever including, insurance, maintenance, licensing, etc.<br />
Program Liability<br />
<strong>Addiction</strong> <strong>Awareness</strong> llc will be responsible for any and all injury or damage as a result of<br />
program’s negligence involving any equipment or service provided under the terms and<br />
conditions, requirements and specifications of the contract. In addition to the liability imposed<br />
upon <strong>Addiction</strong> <strong>Awareness</strong> llc on account of personal injury, bodily injury (including death), or<br />
property damage suffered as a result of <strong>Addiction</strong> <strong>Awareness</strong> llc’s negligence, <strong>Addiction</strong><br />
<strong>Awareness</strong> llc assumes the obligation to save the state of Missouri, including its agencies,<br />
employees, and assigns, from every expense, liability, or payment arising out of such negligent act.<br />
<strong>Addiction</strong> <strong>Awareness</strong> llc also agrees to hold the state of Missouri, including its agencies,<br />
employees, and assigns, harmless for any negligent act or omission committed by any subprogram<br />
