David's participation in this substance abuse evaluation was suggested by a representative from the<br />
criminal justice system. David was charged with a drug crime on one occasion, he has never been<br />
arrested and charged with driving while intoxicated. David was charged with probation/parole<br />
violations on one occasion, he has never been arrested and charged with shoplifting or vandalism.<br />
He was convicted on 2 of these charges. did not successfully complete. David has a one month<br />
history of incarceration. His last incarceration lasted about one month and was for drug charges.<br />
Current Legal Involvement:<br />
In the past 30 days, David was detained/incarcerated on 30 days, he did not engage in any illegal<br />
activities for profit. He is not awaiting any type of charges, trial, or sentencing. He is currently on<br />
probation or parole.<br />
Client Perception of Severity of Legal Problems and Desire for Treatment:<br />
David feels his present legal problems are of moderate importance, and he is moderately interested<br />
in help obtaining legal counseling.<br />
Interviewer Impressions and Recommendations - Legal:<br />
It is my impression that David understood all of the questions, and that he did not deliberately<br />
misrepresent his legal information. He appears to have a moderately severe legal problem, legal<br />
counsel or referral is needed.<br />
FAMILY / SOCIAL SECTION<br />
Marital and Living Situation for Majority of the Past Three Years:<br />
David is currently divorced, and is generally dissatisfied with this situation. He has been divorced<br />
for about 6 years. David has lived with his mate and children for most of the past three years, and<br />
has generally been satisfied with this situation. He has recently been in this living arrangement for 3<br />
months.<br />
Recovery Environment and Social Contacts:<br />
No one residing with him has problems with alcohol or drugs. David currently spends most of his<br />
free time with family, and is generally satisfied with this situation. He reports having a close friend<br />
that he trusts and can rely on.<br />
Relationship Problems Lifetime:<br />
David reports that he has had close, long lasting relationships with family and friends in his<br />
lifetime. In his lifetime, he has had significant problems getting along with his partner, but not with<br />
his children. In his lifetime, he has not had any serious problems getting along with either his<br />
mother or father. He has not had serious problems getting along with other significant family<br />
members in his lifetime. He has not had any significant problems getting along with either siblings<br />
or close friends. He has not had any serious problems getting along with either neighbors or<br />
co-workers. David reports no history of emotional, physical, or sexual abuse in his life.
