Employment History:<br />
He has worked full time for most of the past three years. David's longest full time job lasted 2<br />
years, 6 months. The majority of his employment in the past few years has been skilled manual<br />
work.<br />
Current Financial Resources:<br />
David has not worked or received income from employment in the past 30 days. He did not receive<br />
any income from either unemployment compensation, welfare, pensions, benefits, or social security<br />
in the past month. He has not received any money from family or friends in the past month. David<br />
says that he has not made any money illegally in the past month. David has 4 dependents for whom<br />
he is financially responsible. gfriend brittany - her daughter - child on the way - due in june - cihld<br />
support - 2 years old. Talin and Taya 10/7 - 1 time married - Kylie Chase - 6 years - split up<br />
because i starrted using drugs. relationship with all children. No one contributes any cash, food,<br />
housing, etc. for his support.<br />
Education, Training and Resources:<br />
He completed 12 years of traditional education and received a high school diploma or the<br />
equivalent. He has had no other training or technical education. David has the skill base (finish<br />
carpenter) necessary to acquire a job. He does not have a valid driver's license or a car available.<br />
Client Perception of Severity of Employment Problems and Desire for Treatment:<br />
Overall, he reports experiencing no problems related to obtaining or maintaining employment and<br />
is not troubled or bothered by employment related difficulties. Consequently, help obtaining<br />
treatment for employment related difficulties is not important to David.<br />
Interviewer Impressions and Recommendations - Employment:<br />
It is my impression that David understood all of the questions, and that he did not deliberately<br />
misrepresent this information. He appears to have no need for employment counseling at this time.<br />
