<strong>OSCA</strong> <strong>11</strong>-<strong>036</strong> Treatment Court Specialized Service Providers<br />

<strong>Addiction</strong> <strong>Awareness</strong> will work with clients alternative methods of payment prior to billing drug<br />

court, including but not limited to, working with insurance carriers to gain approval for treatment<br />

services provided. If a third party requires the member to pay any cost‐sharing (such as co‐payment,<br />

coinsurance, or deductible) the treatment court shall pay the cost‐sharing amounts. The treatment<br />

court’s liability for such cost‐sharing amounts shall not exceed the amount the treatment court would<br />

have paid under <strong>Addiction</strong> <strong>Awareness</strong> llc’s price for the service. This <strong>Addiction</strong> <strong>Awareness</strong> LLC may<br />

retain up to one hundred (100) percent of its third party collections if all of the following conditions<br />

exist:<br />

Total collections received do not exceed the total amount of <strong>Addiction</strong> <strong>Awareness</strong> llc's financial<br />

liability for the participant.<br />

There are no payments made by <strong>OSCA</strong> related to fee‐for‐service.<br />

Such recovery is not prohibited by Federal or State law.<br />

Program Services<br />

2.1 Intake/Assessment<br />

The program shall provide a face to face intake session with all participants within seven (7)<br />

calendar days of the date of referral from the treatment court unless otherwise amended and/or<br />

directed by the treatment court. Any exceptions will be documented in the client record.<br />

This <strong>Addiction</strong> <strong>Awareness</strong> LLC’s assessment will be, at a minimum:<br />

Be completed by a qualified substance abuse professional (QSAP),<br />

Include screening for an individuals history of trauma and current personal safety,<br />

Identify information including, but not limited to, name, age, sex, race, sexual orientation<br />

and presenting problem,<br />

Include addiction severity index,<br />

Provide presenting situation,<br />

Contain substance abuse history;<br />

Contain social and family history;<br />

Provide medical evaluation: HIV/STD/TB/ Risk and Service needs,<br />

Contain educational and vocational history,<br />

Contain treatment history including the date, length of stay, outcome and name of the<br />

facility for all psychiatric and substance abuse services, and<br />

Provide recommendation and clinical justification for the level of care of the treatment<br />

services.<br />

<strong>Addiction</strong> <strong>Awareness</strong> LLC will provide a face to face intake session with all participants within seven<br />

(7) calendar days of the date of referral from the Drug Courts unless otherwise amended and/or<br />

directed by the Drug Courts. <strong>Addiction</strong> <strong>Awareness</strong> has averaged a 2 day turn around since receiving<br />

the contract, and goes to the county jail in Buchanan and Andrew to facilitate proper and quicker<br />

referrals of those clients.<br />

Upon admission, <strong>Addiction</strong> <strong>Awareness</strong>, LLC will administer an assessment to each client. This service<br />

will include the following for clients admitted to outpatient programs;<br />

A client’s probation officer will send a referral form to <strong>Addiction</strong> <strong>Awareness</strong>, LLC. Upon receipt of the<br />

form, client will be contacted to set up the initial screening by telephone. At the conclusion of that<br />
