Type II Lepra Reaction • Pink coloured ENL nodules on both hands • Few ENLs show desquamation IRIDOCYCLITIS Patients may present with Iridocyclitis during Type II Lepra reaction. The main symptoms of Iridocyclitis are : • Pain • Redness • Watering of the eyes On examination, circumcorneal congestion with small irregularly shaped pupils which respond sluggishly to light. 54
Difference between Type I Lepra Reaction ( Reversal Reaction ) & Relapse Features 1. Onset 2. Time of onset 3. Old lesions 4. New lesions 5. Ulceration 6. General Condition 7. Response to Corticosteroids 8. Drug Compliance Type I Lepra Reaction 1. Sudden, within a few hours. 2. Generally occurs during treatment or within 6 months of stopping treatment. 3. Become red, swollen and shiny. 4. May develop in some cases. 5. Ulceration sometimes. 6. Fever & malaise unusual. 7. Excellent. 8. May have been good. Relapse 1. Slow and insidious. 2. Generally, occurs a long time after treatment. 3. Margins of some may become red. 4. Few. 5. Unusual. 6. Not affected. 7. Lesions subside but reappear. Corticosteroids are not indicated in relapse. 8. Poor. 55
