A Reproductive Health Needs Assessment <strong>in</strong> <strong>Myanmar</strong><br />
Both progest<strong>in</strong>-only pills and emergency contraception should be <strong>in</strong>cluded <strong>in</strong> the birth<br />
spac<strong>in</strong>g curriculum. Further research to explore the best means of <strong>in</strong>troduc<strong>in</strong>g these<br />
birth spac<strong>in</strong>g methods should be conducted.<br />
Condoms - Condoms have received relatively little attention as birth spac<strong>in</strong>g methods <strong>in</strong><br />
<strong>Myanmar</strong>. It was commonly reported to the <strong>assessment</strong> team that condoms were to be used<br />
primarily for the prevention of sexually transmitted disease. Condoms were often discussed<br />
solely <strong>in</strong> reference to their use <strong>in</strong> commercial sexual encounters. As a consequence, the use<br />
of condoms was stigmatized and many <strong>in</strong>dividuals - men and women - had never seen or<br />
used them.<br />
Information and services for birth spac<strong>in</strong>g should <strong>in</strong>clude discussion of the potential<br />
use of condoms as a contraceptive method. This should <strong>in</strong>volve an emphasis on the<br />
need for correct and consistent use, as well as appropriate technique of use and<br />
disposal.<br />
Information, education, and communications<br />
Despite a trend toward <strong>in</strong>creas<strong>in</strong>g use of birth spac<strong>in</strong>g throughout the country, the <strong>assessment</strong><br />
team observed significant deficiencies <strong>in</strong> knowledge regard<strong>in</strong>g contraceptive side effects,<br />
characteristics of appropriate use, and efficacy. This was particularly evident <strong>in</strong> townships<br />
that did not currently have organized birth spac<strong>in</strong>g programme efforts. It was also reported to<br />
be a serious problem <strong>in</strong> the Shan State, where a significant portion of the population cannot<br />
read <strong>health</strong> educational materials written <strong>in</strong> the <strong>Myanmar</strong> language. The problem is even<br />
greater <strong>in</strong> certa<strong>in</strong> ethnic m<strong>in</strong>ority areas where rates of primary language literacy are low.<br />
Accurate <strong>in</strong>formation concern<strong>in</strong>g the efficacy, side-effects, and appropriate use of all<br />
available contraceptive methods should be made widely available to women and men<br />
through a well-designed IEC strategy us<strong>in</strong>g appropriate media, which must <strong>in</strong>clude<br />
attention to the diversity of languages used throughout <strong>Myanmar</strong>, as well as the <strong>needs</strong><br />
of low literacy population groups.<br />
Reproductive Tract Infections<br />
Programme activities<br />
Primary prevention - The team found that awareness of HIV was high throughout the country<br />
and that most people had fairly accurate knowledge concern<strong>in</strong>g the pr<strong>in</strong>ciple means of HIV<br />
transmission. Knowledge concern<strong>in</strong>g other STDs was more limited and many women did not<br />
dist<strong>in</strong>guish between endogenous <strong>in</strong>fections and STDs as potential causes of vag<strong>in</strong>al<br />
discharge. Evidence suggests that <strong>in</strong> those townships where life skills tra<strong>in</strong><strong>in</strong>g has been<br />
provided through NGOs there has been a beneficial effect on community knowledge,<br />
although <strong>in</strong> some cases the tra<strong>in</strong><strong>in</strong>g participants did little subsequent tra<strong>in</strong><strong>in</strong>g of other<br />
community members.<br />
Health education programmes would be strengthened by community-level counsell<strong>in</strong>g<br />
activities, such as the life skills tra<strong>in</strong><strong>in</strong>g efforts of the MMCWA and MRCS. To be most<br />
effective, these programmes should focus their selection of participants broadly with<strong>in</strong> the<br />
township and on those most likely to engage <strong>in</strong> outreach activities with<strong>in</strong> the community.<br />
