Line 2.<br />
Check the appropriate box to identify the type of<br />
report. If the report was one requested by<br />
either the Commission or an SRO, check the<br />
"Special Call" box and identify who requested<br />
the report by specifying "<strong>CFTC</strong>" or the name of<br />
the SRO.<br />
If the report is a regular<br />
quarterly/semiannual report, but the filing<br />
due date was accelerated by the Commission or<br />
an SRO, check both the "Regular<br />
quarterly/semiannual" and "Special call"<br />
boxes, and note in the "Special Call" box<br />
"Accelerated filing - (SRO's name or <strong>CFTC</strong>)".<br />

Check the appropriate box to identify whether the<br />
report is an initial filing or an amended<br />
report.<br />
Identify the registrant's Designated<br />
Self-Regulatory Organization. This will be<br />
either a commodity exchange or <strong>National</strong><br />
<strong>Futures</strong> <strong>Association</strong>.<br />
Show only those companies which are consolidated<br />
in the <strong>IB</strong>'s <strong>Form</strong> 1-<strong>FR</strong>-<strong>IB</strong>. Do not include<br />
unconsolidated companies. Show the full name<br />
of the company, the <strong>IB</strong>'s percentage ownership<br />
in the company and the company's line of<br />
business. If the <strong>IB</strong> owns both common and<br />
preferred classes of stock, the <strong>IB</strong> should show<br />
individually its interest in each class of<br />
stock.<br />
Attestation<br />
An SRO may require an <strong>IB</strong> to file a separate attestation page with<br />
supplemental schedules it requests. If the <strong>IB</strong> files such separate<br />
attestation page, the <strong>IB</strong>'s Chief Financial or Executive Officer must<br />
nevertheless sign the attestation on the Cover Page to the <strong>Form</strong> 1-<strong>FR</strong>-<br />
<strong>IB</strong> and type or print the signer's name below the signature. If the <strong>IB</strong><br />
is a corporation, the chief executive officer or chief financial<br />
officer must also show his or her corporate title (President, Vicepresident,<br />
Treasurer, etc). The date the attestation was signed must<br />
be shown.
