Cargo Insurance:<br />

4) Cargo Insurance – A. When commodities being transported are subject to<br />

material damage in transportation, proof of Cargo insurance (Form H) is required.<br />

<strong>Oregon</strong> intrastate For-Hire <strong>carrier</strong>s must have a minimum $10,000 cargo insurance<br />

policy on file with ODOT. This commonly applies to <strong>carrier</strong>s hauling general<br />

commodities. ODOT waives this requirement for <strong>carrier</strong>s transporting commodities<br />

not subject to material loss or damage through normal transportation hazards.<br />

B. When transporting commodities that do not require cargo insurance, list the<br />

type of commodity being transported.<br />

A faxed Certificate of Cargo can be used in place of the original in order to process<br />

the Class 1A Permit. The certificate is valid for 30 days and can serve as the<br />

Cargo insurance requirement until the original Form H is filed. (See the<br />

Insurance section for more information.)<br />

5) Application Fee – File the application and the $300 application fee to any<br />

MCTD Registration Office, or mail to: Motor Carrier Transportation Division, 550<br />

Capitol St NE, Salem, OR 97301-2530.<br />

6.) <strong>Oregon</strong> Uniform Cargo Liability Law – <strong>Oregon</strong> law, Chapter 823, requires<br />

that common <strong>carrier</strong>s issue a bill of lading on intrastate shipments and accept<br />

liability for cargo loss or damage. It provides that shippers have at least nine<br />

months after delivery to file claims and two years from the date a claim is denied<br />

to institute a suit.<br />
