What’s New for <strong>20<strong>13</strong></strong><br />
The Health Tradition Health Plan (HTHP)<br />
website, healthtradition.com, was recently<br />
redesigned to offer more functionality<br />
and helpful resources for our members.<br />
Health Tradition members will also be able<br />
to access our Health Risk Assessment tool<br />
via our website. Please check the website<br />
periodically for additional details regarding<br />
health and wellness initiatives through Health<br />
Tradition. Continue to refer to the online<br />
provider directory to make sure the provider/<br />
facility you go to is in the HTHP network for<br />
the state of Wisconsin.<br />
Provider Directory<br />
Go to healthtradition.com. You can select<br />
MMSI Service Center in the bottom right<br />
section of the home page. Once you log<br />
in, click on Find a Doctor across the top tool<br />
bar. Select MMSI as your network and then<br />
you can do a provider search by provider<br />
name, specialty, location, etc. You can also<br />
contact HTHP at (888) 459-3020 to request a<br />
paper copy.<br />
Referrals and Prior Authorizations<br />
You can see any provider in the HTHP<br />
network (primary care or specialist) without<br />
a referral. You must get a referral approved<br />
by HTHP before you see providers outside<br />
the HTHP network (including Mayo Clinic-<br />
Rochester). <strong>Your</strong> doctor must submit a<br />
referral request. Prior authorization is<br />
required for certain services. Contact HTHP<br />
to request a prior authorization. HTHP will<br />
notify you and your provider in writing as to<br />
whether the request has been approved or<br />
denied. For more information, see the HTHP<br />
website or call HTHP at (877) 832-1823.<br />

for mental/behavioral health services. Prior<br />
authorization is required for inpatient care,<br />
group therapy and psychiatric testing.<br />
Dental Benefits (Contact plan for full details.)<br />
Preventive Services: Covered at 100% up to<br />
annual benefit maximum: Exams, cleanings,<br />
fluoride treatments, X-rays and sealants.<br />
Restorative Services: Covered at 80%, up to<br />
annual benefit maximum: Services such as<br />
fillings, bridges, crowns and root canals.<br />
Annual Benefit Maximum: $500 per person<br />
per year on all services.<br />
Orthodontics: None.<br />
Dental Network: You can see any dentist.<br />
Benefits subject to usual and customary<br />
charges unless you use the Health Tradition<br />
Preferred Dental Network. To view our<br />
dental providers, go to healthtradition.com<br />
and click on Provider Directory across the<br />
top toolbar. Scroll down to the heading<br />
titled State of Wisconsin Members and select<br />
Dental Provider Directory.<br />
