We want wellness to be a way of life, so<br />

this year we’ve added incentives to help<br />

you get there! Members who participate<br />

in our wellness programs can earn up to<br />

$150 for a single or $300 for a family. Also,<br />

by completing your health risk assessment<br />

and obtaining your biometrics before the<br />

end of the first quarter you’ll be entered<br />

in a drawing for one of 10 iPad’s that we’ll<br />

be giving away. You can find out more<br />

Referrals and Prior Authorizations<br />

Referrals: Required prior to seeing providers<br />

outside of the network.<br />

Prior authorizations: Required for certain<br />

services. See Security’s Member Handbook<br />

or call customer service for more<br />

information. You or your doctor must submit<br />

the request; Security Health Plan will notify<br />

you in writing of its decision.<br />
