‘In order to insert an implant, the fibrous pocked needs to be opened <strong>and</strong><br />

an artificial pocket in the breast tissue needs to be created,’ Dr Flynn explains.<br />

‘A number of incisions are made on the outside of the fibrous bundle to create<br />

“pleats” <strong>and</strong> exp<strong>and</strong> the space.’<br />

In its most severe form, the tuberous abnormality is seen in all the quadrants<br />

of the breast but there can be different varieties of the condition. For example,<br />

constriction can occur only in the lower portion of the breast, while the top is fully<br />

developed. Similarly, there can be differences between the two breasts – one<br />

breast may be fully constricted, <strong>and</strong> the other normally developed.<br />

‘The surgery is a balancing act, especially when there is unevenness between<br />

the breasts, or within one breast,’ Dr Flynn explains. ‘For example, when the<br />

constriction is only in the lower segment, the surgeon must create an even<br />

balance between the upper <strong>and</strong> lower poles of the breasts. If the condition is<br />

different between both breasts then it becomes about balancing the two.’<br />

Dr Flynn often sees revision patients with tuberous breasts. If the previous<br />

surgeon has been unaware of the condition, <strong>and</strong> fails to address the extra fibrous<br />

tissue, the end results will be unbalanced <strong>and</strong> the implants won’t settle in their<br />

intended position.<br />

The final result of breast augmentation in patients with tuberous breasts can<br />

be quite different than in those patients with naturally formed breast tissue.<br />

‘Because tuberous breasts are tighter <strong>and</strong> more fibrous, there is often not a huge<br />

capacity for great volume,’ Dr Flynn explains. ‘Directly after the augmentation,<br />

the breasts might appear quite tight <strong>and</strong> straight, however this will improve<br />

with time.’<br />

Because the surgery involves disrupting gl<strong>and</strong>ular breast tissue – as opposed<br />

to operating beneath this in regular augmentation surgeries, the recovery period<br />

can be slightly longer for augmentation patients who have tuberous breasts.<br />

There can also be more bruising <strong>and</strong> tenderness post-procedure.<br />

‘It is important to remember that tuberous breasts is a medical condition,’<br />

says Dr Flynn. ‘It is essential that patients with tuberous breasts consult a<br />

skilled surgeon with experience in treating the condition. A unique technique<br />

is required to open the fibrous breast tissue, create an implant pocket <strong>and</strong><br />

produce a balanced, aesthetically-pleasing result. While more complex than a<br />

straightforward augmentation, it can achieve very satisfactory results.’ csbm<br />
