Hospice of the Western Reserve has been providing <strong>care</strong> to<br />

Greater Cleveland since 1978. As a <strong>community</strong>-based, nonprofit<br />

hospice, we’re proud to be able to <strong>care</strong> for those with any lifelimiting<br />

disease, regardless of age, or ability to pay. Care is<br />

provided wherever the patient lives - in their home, in a hospital,<br />

assisted living/long-term <strong>care</strong> facility, or at Hospice House, our<br />

42-bed residential facility overlooking Lake Erie. Our services<br />

include: pain and symptom management, personal <strong>care</strong>, education<br />

for patient and <strong>care</strong>giver, art and music therapies, professional<br />

counseling and social work services, spiritual support, bereavement<br />

services and respite <strong>care</strong> for the <strong>care</strong>giver. For more information,<br />

The Hospice Institute, established in 1997, provides education<br />

about hospice and palliative <strong>care</strong> to health<strong>care</strong> professionals and<br />

students. The Institute collaborates with universities to incorporate<br />

research and end-of-life topics into curricula. Workshops and<br />

conferences cover topics on end-of-life <strong>care</strong>, such as pain and<br />

symptom control, the grieving process, ethical/legal matters and<br />

more.<br />

The Institute provides CEUs for nurses, social workers,<br />

The Elisabeth Severance Prentiss Bereavement Center is a<br />

<strong>community</strong>-based grief support program that provides services to<br />

anyone who has experienced a loss due to death. Services include<br />

counseling for children, adolescents and adults, education and<br />

support groups, crisis response, and wellness programs, as well<br />

as children’s camps, adult retreats, family nights and holiday<br />

programs. The bereavement center also offers <strong>community</strong><br />

education, responds when there has been a death that impacts a<br />

<strong>community</strong>, and assists those who provide support to the bereaved<br />

(health <strong>care</strong> professionals, employers, school personnel, funeral<br />

home staff and emergency/safety forces) through education,<br />

training, consultation, and resources. Special initiatives address<br />

grief in the workplace and school interventions. For more<br />
