use in LRS, particularly given the increasing use <strong>of</strong> elective <strong>induction</strong>. F<strong>or</strong> this reason,<br />

the effectiveness <strong>of</strong> non-pharmacological methods are summarized here.<br />

Alternative methods <strong>of</strong> encouraging the ripening <strong>of</strong> the cervix <strong>or</strong> the onset <strong>of</strong> lab<strong>or</strong> have<br />

been examined through three Cochrane systematic reviews (108-110). The methods<br />

investigated include: mechanical methods (including a balloon catheter (See Figure<br />

3.6a), laminaria tents <strong>and</strong> extra-amniotic infusion), sweeping the membranes, <strong>and</strong> ARM.<br />

The outcome indicat<strong>or</strong>s vary across studies but <strong>of</strong>ten include: vaginal delivery not within<br />

24 hours, Cesarean delivery, hyper-stimulation <strong>of</strong> the uterus with fetal heart changes,<br />

severe neonatal <strong>or</strong> maternal m<strong>or</strong>bidity <strong>and</strong> woman’s satisfaction with the birth<br />

experience. The comparison groups also vary; examples include: placebo <strong>or</strong> no<br />

treatment, prostagl<strong>and</strong>ins <strong>and</strong> <strong>oxytocin</strong>.<br />

In summary, only one small study compared mechanical methods against placebo <strong>or</strong> no<br />

treatment <strong>and</strong> rep<strong>or</strong>ted on vaginal delivery not within 24 hours; the relative risk <strong>of</strong><br />

vaginal delivery not within 24 hours was 0.90 <strong>f<strong>or</strong></strong> mechanical methods versus placebo/no<br />
