are held regularly to correspond with the Personal Health Record Book. For<br />

Service description: The health and well being of children and families in the Wide<br />
Bay District. Community Family Health Nursing and Allied Health Professionals provide<br />
education and support for parents. Within group sessions, growth and development<br />
and relevant parenting information are included in discussion. Community Family<br />
Health is actively involved in Health Promotion and is willing to provide information on<br />
any issue relating to the health of children and adolescents (up to 18).<br />

community and hospital. Referral of clients to appropriate services.<br />
