Service description: Host Home Services offering supported accommodation respite<br />
for families and clients, frail aged, mental health, disabilities, crisis and emergency<br />
care. Private organisation approved with DSQ, no waiting lists, home based family<br />
model of care. Qualified host home staff hold blue cards, first aid and relevant<br />

Service description: This program is funded by Home and Community Care<br />
Program. HACC provides support to assist frail elderly and younger people with<br />
disabilities and their carers. The services provided depend on the needs of the client or<br />
carer ranging from assistance with daily living, general household tasks, transport, in<br />
home respite, personal care, social support, allied health including counselling, social<br />
work and client education and other tasks in response to individual needs. Eligibility:<br />
Persons in the target group at risk of premature or inappropriate placement in<br />
institutional care - screened using the HACC ONI Tool.<br />

Service description: Support to frail aged clients in their own homes, daily if<br />
required - including assistance such as personal care, housework, transport, shopping<br />
and bill paying and assistance with meals. An emergency call system is provided.<br />
Eligibility: is assessed by Aged Care Assessment Service - contact them directly on<br />
4122 8733 if you need this level of support. Fees: 17.5% of basic pension with<br />
adjustments depending on client's financial circumstances.<br />
