Accidents and <strong>in</strong>cidents have been reported to <strong>the</strong> Agency s<strong>in</strong>ce 2006. Each year <strong>the</strong> Agency has received notifications of<br />

at least 150 occurrences <strong>in</strong>vestigated by NIBs. Around 15 % of <strong>the</strong> f<strong>in</strong>al <strong>in</strong>vestigation reports reached <strong>the</strong> Agency by <strong>the</strong><br />

end of <strong>the</strong> year <strong>in</strong> which <strong>the</strong> accident occurred. An overwhelm<strong>in</strong>g majority of f<strong>in</strong>al reports are submitted to <strong>the</strong> Agency<br />

<strong>in</strong> <strong>the</strong> year follow<strong>in</strong>g <strong>the</strong> occurrence of <strong>the</strong> accident. For example, for occurrences <strong>in</strong> 2009, 24 were closed and <strong>the</strong> report<br />

submitted to <strong>the</strong> Agency by <strong>the</strong> end of 2009. Dur<strong>in</strong>g 2010, <strong>the</strong> f<strong>in</strong>al <strong>in</strong>vestigation report was received for 117 out of <strong>the</strong><br />

170 occurrences <strong>in</strong> 2009 (red bars <strong>in</strong> Figure 15). By <strong>the</strong> end of 2010, 28 out of 197 <strong>in</strong>vestigations opened dur<strong>in</strong>g <strong>the</strong> year<br />

were closed, i.e. 14 % of <strong>in</strong>vestigations; this is similar to previous years.<br />
