Thismodel outlines the key components which are necessary in order to improve <strong>and</strong> enhance supports forcaregivers within the <strong>Central</strong> <strong>East</strong> LHIN.PlanningPlanningCollaboratives<strong>Central</strong> <strong>East</strong> <strong>Caregiver</strong><strong>Support</strong>Steering CommitteeCollaborativesPlanningZoneSpecific<strong>Caregiver</strong><strong>Support</strong>ZoneSpecific<strong>Caregiver</strong><strong>Support</strong>ZoneSpecific<strong>Caregiver</strong><strong>Support</strong><strong>Caregiver</strong> support<strong>Central</strong> Coordination SiteRecognitionCoping <strong>Support</strong>RespiteInfo /EducationDaily Living <strong>Support</strong>sFinancial/LegalCare PlanningZoneSpecific<strong>Caregiver</strong><strong>Support</strong>ZoneSpecific<strong>Caregiver</strong><strong>Support</strong>ZoneSpecific<strong>Caregiver</strong><strong>Support</strong>PlanningCollaborativesZoneSpecific<strong>Caregiver</strong><strong>Support</strong>ZoneSpecific<strong>Caregiver</strong><strong>Support</strong>Collaboratives<strong>Central</strong> <strong>East</strong> <strong>Caregiver</strong> <strong>Support</strong> Steering CommitteeThe Steering Committee:• Has a membership of 14, comprised of a CE LHIN representative, caregivers (at least 50%) <strong>and</strong>service providers from representation across the CE LHIN.• It is the first point of contact for the CE LHIN, the three LHIN Health Interest Networks <strong>and</strong> otherstakeholders with regards to caregiver support <strong>and</strong> wellbeing issues.• Provides advice to the CE LHIN on the strategic priorities, goals, <strong>and</strong> objectives/strategies needed toimprove the system of support for caregivers.• Is supported by the <strong>Caregiver</strong> <strong>Support</strong> Coordinator (see outline of job in recommendations forstaffing).• With the <strong>Caregiver</strong> <strong>Support</strong> Coordinator, responds to the recommendations outlined within this reporton supporting caregivers.• Works with the LHIN to review <strong>and</strong> respond to provincial strategies/guidelines which impactcaregivers.• Acts as a resource to guide the work of the <strong>Caregiver</strong> <strong>Support</strong> Coordinator.• Assesses <strong>and</strong> prioritizes the barriers to achieving effective <strong>and</strong> efficient caregiver support.• Wworks with the CE LHIN to develop specific performance indicators which would be built into theMulti Sector Service Accountability Agreements (MSAA) of the organizations funded to deliverservices in zone specific centres.• It is a forum for communication/ collaboration/ knowledge exchange/innovation.31
