Nursing Best Practice GuidelineResponsibility for DevelopmentThe Registered Nurses Association of Ontario (RNAO), with funding from theOntario Ministry of Health and Long-Term Care, has embarked on a multi-year project of nursingbest practice guideline development, pilot implementation, evaluation and dissemination.Screening for Delirium, Dementia and Depression in Older Adults is one of six best practiceguidelines developed in the third cycle of the project. The RNAO convened a panel to developthis guideline, conducting its work independent of any bias or influence from the Ministry ofHealth and Long-Term Care.Purpose and Scope15This guideline has been developed to improve the screening assessment of olderadult clients for delirium, dementia and depression. This guideline does not includerecommendations for the management of these conditions in day-to-day nursing practice.Best practice guidelines are systematically developed statements to assist nurses and clientsin decision making about appropriate healthcare (Field & Lohr, 1990). This guideline focuses on:(1) Practice recommendations: directed at the nurse to guide practice regarding assessmentand screening for delirium, dementia and depression in older adults; (2) Educationrecommendations: directed at educational institutions and organizations in which nurseswork to support its implementation; (3) Organization and policy recommendations: directedat practice settings and the environment to facilitate nurses’ practice; (4) Evaluation andmonitoring indicators.Although this guideline is written to guide nursing practice, geriatric mental healthcare is aninterdisciplinary endeavour. Many settings have formalized interdisciplinary teams and thepanel strongly supports this structure. Collaborative assessment and treatment planningwith the client and family are essential. It is acknowledged that the screening for delirium, dementia and depression needs to bestudied in a more clearly defined way, and that there are gaps in the research evidence. However, this guideline will enable nurses to apply the best available evidence to clinicalpractice, and to promote the most appropriate use of healthcare resources.
