16 The Health Bulletin February, 1955children in North Carolina was providedby general practitioners.We must now bring all physicians toan acute awareness of the facts andfigures as they exist in our own Stateand compare them one by one withthose related here by Dr. Mezera.He has given us the "Whys," and inso many instances the "Hows" in relationto our acknowledged responsibilitiesfor maternal and child health services.We must now supply the "When."We must realize that medical knowledgeis useless unless we are able toapply it for the common good and tomake it known and understood to societyas a whole. The difficulties attimes seem overwhelming, but courageand persistence will bring us to ourgoal:"To let all children enjoy the benefitof scientific advancements and clinicalapplications; to make available a placebewhere every American mother willable to go for advice, to present awidespread campaign to inform themabout available facilities, and to providesufficient hospitals and clinics tomeet everybody's needs." Dr. Mezerahas repeatedly emphasized the idea ofprevention first and then adequate treatment.The ramifications of both aremyriad and are paramount to thepreservation of health and to the savingof lives. Indeed, we are grateful to himfor focusing our thoughts so clearlyand for pinpointing the minute detailsof the over-all program of maternal andchild health. He has even mentionedthe necessity for control measures forthe prevention of German measlesa mere speck on the wide panoramareviewed for us here. Yet we are forcedto assume our own duties in this regard;to evaluate the problem of therisk of congenital malformation andstillbirths resulting from maternal infectionduring the first trimester; tolearn to diagnose the disease accurately;to properly evaluate the role ofGamma Globulin in preventing rubella;and finally, to make up our mindswhether or not to institute a moreeffective program for attacking therubella problem by deliberate exposureof girls to the disease before the childbearingage, with rigid control necessaryto avoid exposing gravidas.It occurs to me that there is oneconcept which might be advantageouslyincluded in the general foundation onwhich adequate maternal and childwelfare programs must be built.Why not include in our educationalcampaign the dissemination of newlygained medical information and ideason the probable desirability of familyplanning and birth control under thesepossible tragic conditions:1. In those instances where childbearingis definitely detrimental to thehealth of the mother; where there isa possibility of diminishing her activitiesat home, or of removing her fromthe family circle completely?2. The prevention of the birth ofanother child in a family where thereis hemophilia, epilepsy, familial idiocyor muscular dystrophy?Isn't it a part of our preventivemedical and health program to presentthe consequences of further familyexpansion to those non-comprehendingparents who are faced withthese tragic conditions?We can see how the ideas and wordsof our speaker serve to stimulate awhole new, broader segment of activitywhich must necessarily fit and dovetailinto the general plan for maternaland infant care. He has given us ideas,has made us want to start movingand has even provided us with mapsto follow. What we need now is to"Hitch up old Bess and get down theroad."
