Secondary cases <strong>an</strong>d chemoprophylaxisINTRODUCTIONThe household contacts of a p<strong>at</strong>ient with meningococcal disease (meningitis <strong>an</strong>d/orsepticemia) have <strong>an</strong> increased risk of contracting the disease,I -) To protect thesehousehold contacts against secondary disease, chemoprophylaxis is recommended inthe United St<strong>at</strong>es <strong>an</strong>d Gre<strong>at</strong> Britain.ofS The objective of this measure is to elimin<strong>at</strong>emeningococci from the nasopbarynx of carriers, by which the further tr<strong>an</strong>smission ofthe meningococcus within the household is interrupted. It is assumed th<strong>at</strong> this willprevent secondary cases of meningococcal disease (MD). The most preferred drug forchemopropbylaxis is rifampicin. Altern<strong>at</strong>ive drugs are minocycline. ceftriaxone <strong>an</strong>dciprofloxacin. 111cse <strong>an</strong>tibiotics are capable of elimin<strong>at</strong>ing meningococci from thenasopharynx (efficacy as compared to placebo: 79-100%).6-' Because most secondarycases occur within 5 days after hospital admission of the primary p<strong>at</strong>ient,! tbe usualrecommend<strong>at</strong>ion is to prescribe the prophylaxis to the household contacts as 50011 aspossible after diagnosis." 5 Because most <strong>an</strong>tibiotics which are used for tre<strong>at</strong>ment ofgeneralized MD do 110t reach sufficient salivary concentr<strong>at</strong>ions, it is recommended togive prophylaxis to the primary p<strong>at</strong>ient as well, prior to the p<strong>at</strong>ient's discharge fromhospital, to ensure th<strong>at</strong> carrier strains are not reintroduced into the household by theprimary p<strong>at</strong>ient.' so Medical personnel th<strong>at</strong> has been in close contact with a p<strong>at</strong>ientwith MD (mouth-to-mouth resuscit<strong>at</strong>ion), should also be prophylactically tre<strong>at</strong>ed."In the Netherl<strong>an</strong>ds the prescription of chemoprophylaxis is not accepted practice.In 1980 the N<strong>at</strong>ional Health Council preferred close monitoring of the health-st<strong>at</strong>usof contacts of a primary p<strong>at</strong>ient, <strong>an</strong>d left the prescription of chemoprophylaxis to theopinion of the <strong>at</strong>tending physici<strong>an</strong>." Apart from the advoc<strong>at</strong>ion of rifampicin, nofurther instructions were provided with regard to chemoprophylaxis. In 1988 theprescription of chemoprophylaxis with rifampicin to the household contacts of aprimary p<strong>at</strong>ient with MD was recommended in the Du tch journal "Nederl<strong>an</strong>dsTijdschrift voor Geneeskunde", but in a subsequent paper in this journal no unifiedpolicy could be established with regard to chemoprophylaxis." 12 In 1990 it wasconcluded, again in this journal, th<strong>at</strong> there was too little evidence of the beneficialeffect of chemoprophylaxis with rifampicin to justify its prescription." In the protocol"Infectieziekten" issued by the Department of the Chief Medical Officer of Health(CM OH), to which the Dutch Public Health Services refer, chemoprophylaxis withrifampicin is suggested, but not officially recommended. J4During the period from 1st April 1989 to 30th April 1990, a n<strong>at</strong>ion-wide survey onMD was conducted in the Netherl<strong>an</strong>ds. Secondary study-objectives were the assessmentof the secondary <strong>at</strong>tack r<strong>at</strong>e (SAR) of MD among the household contacts ofprimary p<strong>at</strong>ients with rvrn, <strong>an</strong>d the inventariz<strong>at</strong>ion of the applic<strong>at</strong>ion of chemoprophylaxisill the Netherl<strong>an</strong>ds. In this article we present the results of tbis part ofthe survey, <strong>an</strong>d advoc<strong>at</strong>e a reappraisal of the recommend<strong>at</strong>ion of chemoprophylaxis inthe Netherl<strong>an</strong>ds.77
