GuarnEPATitle V Federal Operating <strong>Permit</strong>Layon Municipal <strong>Solid</strong> <strong>Waste</strong> Landfill, Inarajan, <strong>Guam</strong><strong>Permit</strong> No. FO-OOSlIE.Startu,p, Shutdown, and Malfunction PlanII.E.1 The permittee shall have in place a written Startup, Shutdown, and Malfunction(SSM) plan according to the provisions in 40 CFR 63.6(e)(3). A copy of the SSMplan must be maintained on site. Failure to write, implement, or maintain a copyof the SSM plan is a deviation from the requirements 40 CFR 63, Subpart AAAA.[40 CFR §63.1960]II.F.Monitoring and Testing Requirements1I.F.1 The permittee shall install a sampling port and a thermometer, other temperaturemeasuring device, or an access port for temperature measurements at eachwellhead, and:a. Measure and record the gauge pressure at each individual well on amonthly basis. If a positive pressure exists, action shall be initiated tocorrect the exceedance within 5 calendar days, except for the threeconditions allowed under 40 CFR §60.753(b). Ifnegative pressure cannotbe achieved without excess air infiltration within 15 calendar days ofthefirst measurement, the gas collection system shall be expanded to correctthe exceedance within 120 days of the initial measurement ofpositivepressure. Any attempted corrective measure shall not cause exceedancesof other operational or performance standards. An alternative timeline forcorrecting the exceedance may be submitted to USEPA and GEPA forapproval. The <strong>Permit</strong>tee is not required to expand the system during thefirst 180 days after gas collection system startup;b. Monitor and record the nitrogen or oxygen concentration in the landfillgas on a monthly basis as provided in 40 CPR §60.753(c). If a wellexceeds one of these operating parameters, action shall be initiated tocorrect the exceedance within 5 calendar days. If correction oftheexceedance cannot be achieved within 15 calendar days of the firstmeasurement, the gas collection system shall be expanded to correct theexceedance within 120 days of the initial exceedance. Any attemptedcorrective measure shall not cause exceedllllces of other operational orperformance standards. An alternative timeJine for correcting theexceedance may be submitted to USEPA and GEPA for approval; andc. Monitor and record the temperature of the landfill gas on a monthly basis.[40 CFR §60.756(a), §60.755(a)(3), §60.755(a)(5)]1I.F.2 In seeking to comply with 40 CFR §60.752(b)(2)(iii) using an open flare, the<strong>Permit</strong>tee shall install, calibrate, maintain, and operate according to themanufacturer's specifications the following equipment:12 of 39
