GuarnEPATitle V Federal Operating <strong>Permit</strong>Layon Municipal <strong>Solid</strong> <strong>Waste</strong> Landfill, Inarajan, <strong>Guam</strong><strong>Permit</strong> No. FO-005f. The operating conditions as existing at the time of sampling ormeasurement.III.D. Reporting RequirementsII1.D.I As required in Section III.H and in conjunction with the requirements of SectionIII.! of this permit the permittee shall report annually the total tons per yearemitted of each regulated air pollutant, including hazardous air pollntants. TherepOlting of annual emissions is due within sixty (60) days following the end ofthe each calendar year. Upon the written request of the permittee, the deadline forreporting of annual emissions may be extended, if GEPA determines thatreasonable justification exists for the extension. [GAPCSR, Section 1104.24(c)]II1.D.2 The permittee shall report in writing within thiity (30) days the modification,relocation, discontinuance of operation of dismantlement of the landfill gas flareidentified in Condition 11.A. I ofthis permit. [GAPCSR, Section 1104.5(a)]m.D.3 The permittee shall provide a report to GEPA of the results of all monitoring andrecordkeeping required by this permit at least once every six (6) months, startingfrom the date of issuance ofthis permit. [GAPCSR, Section 1104.12(7)(1)]II1.DA The permittee shall submit to GEPA and USEPA Region 9 repOlts of anymonitoring required under this permit for each six month reporting period (orfraction thereof) from January I to June 30 and from July I to December 31. AllrepOlts shall be submitted to USEPA and shall be postmarked by the 30th dayfollowing the end of the reporting period. All instances of deviations from permitrequirements must be clearly identified in such reports. All required reports mustbe certified by a responsible official consistent with Condition IV.E of this permit.[40 CFR §71.6 (a)(3)(iii)]a. The monitoring repOlt must include the following:(1) The company name and address,(2) The beginning and ending dates of the repOlting period,(3) The emissions unit or activity being monitored,(4) The emissions limitation or standard, including operationalrequirements and limitations (such as parameter ranges), specifiedin the permit for which compliance is being monitored,(5) All instances of deviations from permit requirements, includingexcursions or exceedances as defined under 40 CFR part 64, andthe date on which each deviation occurred,220f39
