26<strong>Update</strong> <strong>in</strong> <strong>Anaesthesia</strong>adm<strong>in</strong>istration of fleca<strong>in</strong>ide 50 - 100mg slowly ivmay restore s<strong>in</strong>us rhythm. However fleca<strong>in</strong>ideshould only be used where the arrhythmia is lifethreaten<strong>in</strong>g and no other options are open. Itshould be avoided if left ventricular function is pooror there is evidence of ischaemia. Beta blockers are sometimes used to control theventricular rate but may precipitate heart failure <strong>in</strong>the presence of an impaired myocardium,thyrotoxicosis or calcium channel blockers, andshould be used with caution.2. Chronic AF with a ventricular rate of greater than100/m<strong>in</strong>. Aim to control the ventricular rate to less than100/m<strong>in</strong>ute. This allows time for adequate ventricular fill<strong>in</strong>gand helps ma<strong>in</strong>ta<strong>in</strong> the cardiac output.Digitalisation - if patient not already tak<strong>in</strong>g it.Consider extra digox<strong>in</strong> if not fully loaded - bewaresigns of digox<strong>in</strong> toxicity, nausea, anorexia,headache, visual disturbances etc, and arrhythmiasespecially ventricular ectopics and atrialtachycardia with 2:1 block. Beta blockers or verapamil AmiodaroneWhen AF has been present for more than a few hoursanticoagulation is necessary before DC cardioversion toprevent the risk of embolisation. Usually patients shouldbe warfar<strong>in</strong>ised for 3 weeks prior to elective DCcardioversion, with regular monitor<strong>in</strong>g of their prothromb<strong>in</strong>time. An INR of 2 or more is a satisfactory value at whichto proceed with cardioversion. Warfar<strong>in</strong> should then becont<strong>in</strong>ued for 4 weeks afterwards. Occasionally when apatient develops AF and is compromised by it, DCcardioversion has to be considered even whereanticoagulation is contra<strong>in</strong>dicated (eg recent surgery).ATRIAL ECTOPIC BEATS (figure 13)An abnormal P wave is followed by a normal QRScomplex. The P wave is not always easily visible on theECG trace. The term ‘ectopic’ <strong>in</strong>dicates that depolarisationorig<strong>in</strong>ated <strong>in</strong> an abnormal place, ie not the SA node hencethe abnormal shape of the P wave. If such a focusdepolarises early the beat produced is called anextrasystole or premature contraction and may be followedby a compensatory pause. If the underly<strong>in</strong>g SA node rateis slow, sometimes a focus <strong>in</strong> the atria takes over and therhythm is described as an atrial escape, as it occurs after asmall delay. Extrasystoles and escape beats have the sameQRS appearance on the ECG, but extrasystoles occurearly whereas escape beats occur late.Causes: Often occur <strong>in</strong> normal hearts May occur with any heart disease Ischaemia, hypoxia Light anaesthesia Sepsis Shock Anaesthetic drugs are common causesManagement: Correction of any underly<strong>in</strong>g cause. Specific treatment of atrial ectopic beats isunnecessary unless runs of atrial tachycardia occur- see above.BROAD COMPLEX ARRHYTHMIASVentricular Ectopic Beats (figure 14)Depolarisation spreads from a focus <strong>in</strong> the ventricles byan abnormal, and therefore slow, pathway so the QRSEctopic beatFigure 13: Atrial Ectopic Beats
