<strong>Update</strong> <strong>in</strong> <strong>Anaesthesia</strong> 57The sciatic nerve leaves the pelvis and enters the buttockthrough the greater sciatic foramen, and then passes slightlymedial to the midpo<strong>in</strong>t between the greater trochanter andthe ischial tuberosity, ly<strong>in</strong>g just posterior to the hip jo<strong>in</strong>t. Itcan be blocked at several po<strong>in</strong>ts along this course (seetechniques). The sciatic nerve leaves the buttock, pass<strong>in</strong>gout from under the lower border of gluteus maximus muscleand runs distally down the thigh to the popliteal fossa.Areas supplied by the <strong>in</strong>dividual nerves:A pictorial illustration of the sk<strong>in</strong> areas supplied is given <strong>in</strong>figure 1.Femoral nerve: supplies sk<strong>in</strong> over the anterior (front) ofthe thigh, the anterior knee, some of the medial (<strong>in</strong>ner)thigh, via the saphenous nerve it also supplies the anteromedialaspect of the calf down to and <strong>in</strong>clud<strong>in</strong>g the medialmalleolus. It has branches to the hip jo<strong>in</strong>t and knee jo<strong>in</strong>tand supplies much of the shaft of the femur. Sensation tothe medial aspect of the big toe may come from thesaphenous (femoral nerve).Lateral cutaneous nerve of the thigh: supplies sensationto the sk<strong>in</strong> over the lateral (outside) thigh, from the greatertrochanter to the knee, and on to the anterior thigh.Figure 1: Cutaneous nerve distribution of the lower limb.
