GUADALUPE COUNTYProgram Name:Status:Type:Program Description:Community Connections ProgramNewWrap-AroundThe Community Connections Program maintains three components to addressjuvenile needs in <strong>the</strong> community. Component # 1 is designed to provide effective casemanagement services that prevent at-risk youth <strong>from</strong> repeat violations that couldresult in detention. Youth and families are provided with a unique combination ofsupport services designed to address <strong>the</strong> needs of <strong>the</strong>ir specific circumstances,discourage future acts of delinquent behavior, and promote positive youthdevelopment.The level of service provided is tailored to <strong>the</strong> individualized need of each youth andfamily referred to <strong>the</strong> program. The Community Connections Program addresses <strong>the</strong>multiple contributing factors to a youth's potential detention including, but not limitedto multiple violations, failure to attend school, and curfew violations. Contractor'scasework staff works in partnership with <strong>the</strong> youth and family to address andovercome <strong>the</strong>se factors, ultimately supporting <strong>the</strong>ir efforts to lead law-abiding andproductive lives.Youth referred to <strong>the</strong> Community Connections Program have low to medium risklevels, indications of mental health and substance abuse issues and a violation ofconditions of probation. Primary reasons for <strong>the</strong>ir violating juvenile probation includetruancy, curfew violations, and substance abuse.Component #2 provides early identification of needs that could require residentialplacement in a different community based program with <strong>the</strong> placement goal beingreturning to <strong>the</strong> home community, without commitment to <strong>the</strong> <strong>Texas</strong> Youth<strong>Commission</strong>. Such placement into community based residential services allows forCourt monitored placement and supervision while continuing to address <strong>the</strong> needs of<strong>the</strong> juvenile.HARDIN COUNTYProgram Name:Status:Type:Program Description:<strong>Juvenile</strong> Abusive BehaviorNewLife SkillsThis program is comprehensively addressing all issues effecting adolescents such asemotionally and physical abusive behavior, inappropriate sexual behavior, self-control,character education, substance abuse prevention, communication skills, truancyissues, <strong>the</strong>ft prevention, teen parenting (for juveniles), parenting teens (forparents)which includes a "responsible-use" plan between each.
