- 87 -- steps to be taken at both establishments visited to draw up an individual treatment plan foreach psychiatric patient, composed of both pharmacotherapy and a wide range ofrehabilitative and therapeutic activities, including the goals of the treatment, the therapeuticmeans used and the staff members responsible (paragraph 141);- steps to be taken to ensure that all patients at the Secure Unit of Nubarashen PsychiatricHospital and the Nork Centre of Mental Health whose health so permits have access to onehour of outdoor exercise per day. Further, immediate steps should be taken to improve theconditions under which patients take outdoor exercise at Nubarashen Psychiatric Hospital(paragraph 142).comments- the <strong>Armenia</strong>n authorities are invited to explore possibilities for granting leave to patients atthe Nork Centre of Mental Health, to assist with rehabilitation and to counter the adverseeffects of hospitalisation (paragraph 143).Means of restraintrecommendations- the <strong>Armenia</strong>n authorities to ensure that the application of mechanical means of restraint to apatient does not take place in the sight of other patients, unless the patient concernedexplicitly requests otherwise or is known to have a preference for company. Means ofrestraint should be applied to a patient in a room specially designed for that purpose andstaff should not be assisted by other patients when applying means of restraint. Once themeans of restraint have been removed, a debriefing of the patient should take place(paragraph 145).comments- the <strong>CPT</strong> trusts that the <strong>Armenia</strong>n authorities will ensure that all instances of restraint aresystematically recorded in the new registers established for that purpose (paragraph 144).Safeguardsrecommendations- “forensic” patients to be systematically informed of the decision of the psychiatriccommission and the court decision (and be given a copy of these documents), as well as ofthe legal remedies available to challenge them. Further, legal assistance to such patientsshould be ensured (paragraph 146);
