IntroductionWhat is D&T Sub-Committee ?D&T sub-committee is a sub-committee under theMedical Advisory Committee (MAC). It evaluatesthe clinical use of medication, develops policies formanaging medication use and administration, andmanages the formulary. The committee has broadresponsibilities in determining which medicationwill be made available, and how it will be used.What are the functions of D&T Sub-committee?• Advising medical, administrative, and pharmacydepartments on medication-related issues.• Developing medication policies and procedures.• Evaluating and selecting medication for theformulary.• Assessing medication use to identify potentialproblems.• Developing and implementing an efficient andcost-effective formulary.• Ensure that only efficacious, safe, cost-effectiveand good quality medication are used.Medication that requires D&T Sub-Committeeapproval:• New medication that is not in the hospital formulary.• New formulation/dosage form for medication currentlyin hospital formulary.• New prescribing indication/criteria for drug currentlyin hospital formulary.

B. PROCEDURE TO REQUEST FORNEW DRUG TO BE INCLUDED INHOSPITAL FORMULARYSTEP 1: Download form BK-PHS-014 from PortalPPUM http://portal.ummc.edu.my. Log inusing SingleSignOn ID and password -> goto “Menu” ->Click “Muat Turun Borang” ->scroll down to search for “Borang JabatanFarmasi” -> click on “Request for newmedication to be included in UMMC formularyBK-PHS-014 ”.STEP 2: Fill up the BK-PHS-014 form. To be completedby prescriber only.STEP 3: Email a PDF copy to Head of PharmacyDepartment (kj_phs@ummc.edu.my).Add Cc to: zuraini@ummc.edu.myyywong@ummc.edu.myORhand in ONE printed copy to:Pharmacy Office,Pharmacy Department,1st Floor, Menara Utama,(Attn: Wong Yin Yen)STEP 4: Discuss medication request in D&TSub-Committee Meeting.C.
