A GP SERVICES COMMITTEE INCENTIVES 3.6 COMPlEx CARE PAyMENTS EFFECTIVE JANUARy 1, 2008 The revised single option pays a maximum of $315 per calendar year (January to December) for the complex care plan and annual complexity. Complex Care Billing Summary 14033 – Annual complex care management fee $315 (paid in one lump sum) • First visit (to create the major complex care plan) must be in person. • Follow-up visits may be conducted in person, by telephone, or by email. • Care provided in the office is paid on a traditional fee-for-service basis billing age-appropriate fees for the service provided • CPX fee (00101/16101/17101/18101) may be billed at any time during the calendar year. 14039 – Complex care telephone/email follow-up management fee $15 • Once the complex care plan has been created and the complex care management fee (14033) has been successfully billed, the GP practice may access a new phone/email follow up – two-way communication free a maximum of four times per year • The complex care telephone/email follow-up care may be provided by a health professional other than a GP, e.g., a nurse. A GP SERVICES COMMITTEE INCENTIVES • CDM fees 14050/14051/14052 are payable on the same day for the same patient if all other criteria are met. • Community conferencing fee 14016 payable on the same day for the same patient if all other criteria are met. • Facility conferencing fee 14015 only payable on the same day for the same patient if a community-based patient is temporarily in a facility (e.g., sub-acute care unit) when both the complex care planning and conferencing occur and if all other criteria are met. • Cardiovascular risk assessment fee 14034 payable on the same day for the same patient if all other criteria are met for complex patients who live at home or in an assisted-living facility (excluding long-term care facilities). • Out-of-office fees can be billed in addition (provided criteria have been met).
