B. COLLEGE OF PHYSICIANS AND SURGEONS OF BC<br />

The College of Physicians and Surgeons of BC, with support from the BC Ministry<br />

of Health, has developed interactive small-group workshops on Chronic Disease<br />

Management with one physician-facilitator per six participants. These workshops are<br />

designed for groups of six to seven general practitioners and are half-day (3.5 hours) in<br />

duration; they can be held morning, afternoon, or evening. The goals of this program are:<br />

• to assist general practitioners who wish to introduce strategies into their<br />

practices<br />

• to help GPs use their own patient records, to compare "usual care"<br />

of selected chronic diseases with current clinical practice guideline<br />

recommendations<br />

• to discuss in a facilitated small group how to overcome barriers to good CDM<br />

• to provide hands-on exploration of PDA applications for CDM<br />

• to assist GPs in using a patient registry of chronic diseases, on paper, with EMR,<br />

or with the CDM Toolkit<br />

• to help GPs to use flow sheets for chronic diseases such as those currently<br />

reimbursed by MSP<br />

• to discuss the role of CDM in primary care reform<br />
