How do I access my probabilistic registers?<br />

The ministry has made available each doctor’s register for diabetes and congestive<br />

heart failure (CHF), and more registers will be available soon. To obtain these registers,<br />

submit a request for the <strong>Practice</strong> Support Program’s Physician Profile Analysis Report.<br />

See section D. below for details.<br />

D: PRACTICE SUPPORT PROGRAM PHYSICIAN PROFILE<br />

ANALYSIS REPORT<br />

This BC Ministry of Health report provides an analysis of individual physician’s practice<br />

demographics that highlight’s the practice’s chronic disease patients, billing patterns<br />

for chronic disease incentives, and proxy measures for quality of care. This report<br />

includes the provincial probabilistic registers for various conditions – currently diabetes<br />

and CHF, with other registers available soon.<br />

The request form is available at the end of this section, or may be downloaded from<br />

A sample of the report for a fictional doctor and patients is also viewable online at:<br />

The ministry sends this report securely to physicians via CD or through the secure<br />

web site (https://healthregistry.moh.hnet.bc.ca/). This report provides a simple means<br />

to get started with developing patient registers for chronic disease patients and<br />

helps physicians engage in planned, proactive care. It is free, secure, confidential,<br />

and available by request to all practising family physicians in BC. The report provides<br />

print and electronic formats of the registers. To help the doctor to get started with<br />

an electronic register, if the doctor chooses, the electronic format of the register is<br />

suitable for import to the CDM Toolkit, MS Excel, or other products. Future versions of<br />

the report will also be suitable for import into PITO-approved EMRs.<br />

E. CDM TOOLKIT<br />

The CDM Toolkit contains the electronic versions of the flow sheets for CHF, diabetes,<br />

and hypertension, and a variety of flow sheets for other chronic conditions. You may<br />
