As a result, the case definition for a condition may be either too sensitive or specific, i.e., catching some patients who do not have the condition and excluding some who do. Second, there is a time lag between a patient’s encounter with the health system and the end of the year when the data is complete and available for analysis. For instance, administrative data for the fiscal year ending March 31 is generally not available until the fall of the same year and, in the interim, patients may die, move away, move into a practice, be newly diagnosed, or be rediagnosed with a different condition that would change their status on the register. Despite these limitations to the administrative data, probabilistic registers are a good source of condition-specific information for use by physicians, health authorities, and the Ministry of Health for the following purposes. • To estimate performance; for example, the proportion of patients with diabetes who receive tests such as A1C in a year. Measurement of the changes in these indicators over time and variation across the province can be very useful when planning and evaluating programs intended to improve care. Physicians have also begun to use their own performance data to monitor and improve their care of patients according to evidence-based clinical guidelines. • To estimate the prevalence of certain conditions across BC or other geographic areas or practice settings and to monitor trends and care requirements over time. For individual physicians, registers can also help to identify the patients in their practice with a particular condition. Their key value is that they provide a relatively current list of patients that can be used by doctors as an aid to building their own practice registers. The ministry has made available each doctor’s register for diabetes and congestive heart failure (CHF), and more registers will be available soon. To obtain these registers, submit a request for the Practice Support Program’s Physician Profile Analysis Report. See section D. below for details. D: PRACTICE SUPPORT PROGRAM PHYSICIAN PROFILE ANALYSIS REPORT This BC Ministry of Health report provides an analysis of individual physician’s practice demographics that highlight’s the practice’s chronic disease patients, billing patterns for chronic disease incentives, and proxy measures for quality of care. This report includes the provincial probabilistic registers for various conditions – currently diabetes and CHF, with other registers available soon. The request form is available at the end of this section, or may be downloaded from the Practice Support Program web site at: http://www.impactbc.ca/files/documents/ Request_Report_letter.pdf A sample of the report for a fictional doctor and patients is also viewable online at: http://www.impactbc.ca/files/Physician_Profile_Analysis_Sample_Report.pdf The ministry sends this report securely to physicians via CD or through the secure web site (https://healthregistry.moh.hnet.bc.ca/). This report provides a simple means to get started with developing patient registers for chronic disease patients and helps physicians engage in planned, proactive care. It is free, secure, confidential, and available by request to all practising family physicians in BC. The report provides print and electronic formats of the registers. To help the doctor to get started with an electronic register, if the doctor chooses, the electronic format of the register is suitable for import to the CDM Toolkit, MS Excel, or other products. Future versions of the report will also be suitable for import into PITO-approved EMRs. E. CDM TOOLKIT The CDM Toolkit contains the electronic versions of the flow sheets for CHF, diabetes, and hypertension, and a variety of flow sheets for other chronic conditions.
