L . 1 3 . I m m u n i z a t i o n R e q u i r e m e n t sState <strong>and</strong> Federal law require the <strong>College</strong> to retain certain medical records on each student. This medicalinformation is confidential <strong>and</strong> remains in the student’s file. Each student must complete the required sections<strong>of</strong> the <strong>Student</strong> Immunization Record by the 45th day <strong>of</strong> the semester or will not be allowed to attend class.The <strong>Delaware</strong> State Board <strong>of</strong> Communicable Disease Regulations (Section 7.194) m<strong>and</strong>ates that all studentsborn after 1956 provide an accurate vaccination history showing immunity to measles, mumps <strong>and</strong> rubella.Beginning in 2001, the State <strong>of</strong> <strong>Delaware</strong> requires all colleges <strong>and</strong> universities to provide information toadmitted students about Meningococcal Meningitis <strong>and</strong> notice <strong>of</strong> the availability <strong>and</strong> benefits <strong>of</strong> thevaccination. It is the student’s or in the case <strong>of</strong> a minor, a parent/guardian’s decision, to be vaccinated.<strong>Student</strong>s must supply the <strong>College</strong> with either the date <strong>of</strong> their vaccination or sign a waiver form, indicating thatthey have decided to not be vaccinated. <strong>Student</strong>s are not permitted to attend class without this information onfile.In addition, the <strong>College</strong> requires students to provide the date <strong>of</strong> vaccination, within 10 years, <strong>of</strong> their mostrecent Tetanus-Diphtheria vaccination. The student is also required to provide information relating toTuberculosis if they identify with the categories outline on the Immunization Form.L . 1 4 . C h a n g i n g M a j o r sSelecting a major is a serious <strong>and</strong> possibly difficult decision. <strong>Student</strong>s’ goals <strong>and</strong> interests may change beforetheir studies are completed. When considering a change <strong>of</strong> major, students should discuss their intentions withtheir Area Coordinator, the Director <strong>of</strong> <strong>Student</strong> Services or the Dean. Consider the options carefully. A change<strong>of</strong> major will usually require extra time in school since the curriculum at DCAD is very different fromdepartment to department. Upon deciding to change major a “Change <strong>of</strong> Major Form” must be completed <strong>and</strong>returned to the Registrar. “Change <strong>of</strong> Major Forms” are available from the Registrar’s Office.L . 1 5 . C l a s s E v a l u a t i o n sToward the end <strong>of</strong> each semester students are asked to complete class evaluation forms. The evaluationprocess is a method <strong>of</strong> obtaining constructive feedback from students. It is therefore important that studentsbe honest <strong>and</strong> thoughtful in their evaluation <strong>of</strong> course content <strong>and</strong> teaching methodology. Faculty members donot have access to the evaluations until after their grades have been turned in therefor students areguaranteed complete anonymity.L . 1 6 . E m e r g e n c y N o t i f i c a t i o n S y s t e m<strong>Delaware</strong> <strong>College</strong> <strong>of</strong> <strong>Art</strong> <strong>and</strong> Design, like all other academic institutions, is responsible for delivering emergencynotifications to all students in an expedited time frame. In order to deliver this information quickly <strong>and</strong>efficiently, DCAD’s Emergency <strong>and</strong> Event Notification System allows for the timely distribution <strong>of</strong> information,including non-emergency yet vital information (ie. course registration dates, college closing, etc.) <strong>and</strong> non -emergency event information (ie. common hour lectures, on-campus activities, etc.). <strong>Student</strong>s are required toprovide the Office <strong>of</strong> <strong>Student</strong> Services with email <strong>and</strong>/or telephone contact information for the purposes <strong>of</strong>this notification system.34
