General Mental Health and Mood and Anxiety Disorders Clinics: Through theGMHC and MADC, the Fellow will gain experience in evaluating and treating a widevariety of Axis I mental health disorders. A primary focus of the GMHC is theprovision of services to veterans with a variety of psychological disorders who couldbenefit from time-limited treatment (e.g., veterans with adjustment disorders,veterans dealing with loss and bereavement, veterans with multiple mental healthissues that are not best treated in a specialty clinic). The MADC emphasizesdifferential diagnosis of mood disorders and non-PTSD anxiety disorders, andevidence-based treatment of these disorders. It is the primary referral clinic forveterans struggling with significant depressive, manic, or non-PTSD anxietysymptoms. The Fellow will complete diagnostic and suicide risk assessments.Assessments include structured clinical interviews (e.g. SCID) and standardizedself-report assessments. Treatments are primarily evidence-based (e.g., cognitivebehavioralapproaches) and involve both individual and group modalities.Urgent Care Clinic: The Fellow will conduct evaluations in the Urgent Care Clinic,providing a unique opportunity for training in risk assessment and stabilization ofacute psychiatric crises. This experience will provide the Fellow with exposure topatients with a broad range of psychopathological conditions, including both acuteand sub-acute symptoms (e.g. psychotic disorders, alcohol and drug intoxication).While in the UCC, the Fellow will gain additional experience in consultation, as wellas navigation of complex systems issues (e.g., coordinating voluntary orinvoluntary hospitalization, mandatory reporting.)VA Boston Psychology FellowshipIntegrated Primary Care Behavioral Health: The PCBH offers co-located,immediate, collaborative mental healthcare within the primary care context. TheFellow will receive training in psychological assessment and brief treatment ofveterans with a broad range of clinical conditions who present to primary care (e.g.,depression, anxiety, trauma, substance use disorders). Evaluation and treatmentin the PCBH is necessarily brief, with a focus on identifying key issues of concern tothe primary care patient. Standardized self-report measures inform the clinicalassessment. The Fellow will learn to develop and carry out short-term, evidencebasedbehavioral treatment plans (e.g., cognitive behavioral treatment,motivational enhancement). Mental health care is integrated into existing primarycare treatment, and thus communication and liaison with the Primary Care team isessential. Fellows will develop proficiency in providing individual consultation toprimary care staff on mental health issues and management of suicide risk.Teaching Methods: Fellows will receive training using multiple modalities,including individual and group supervision, didactic trainings (clinic-specific andFellowship-wide), case conferences and rounds. Live supervision is provided in theUCC. The Fellow will participate in multidisciplinary GMH/MADC and PCBH team19
meetings that may include psychologists, psychiatrists, social workers, clinicalnurse specialists, primary care providers, and other trainees (e.g., psychologyinterns, practicum students, and a psychiatry resident).Supervision: Dr. Melanie Vielhauer is the Director of the GMH Clinic and serves asthe fellowship Track Director. She also oversees mental health services provided inVA Boston’s PCBH clinics. Dr. Vielhauer and Dr. Barbara Kamholz co-direct theMADC. Together, Drs. Vielhauer and Kamholz provide the leadership for thepostdoctoral training program. Additional supervision will be provided to thepostdoctoral fellow by other faculty affiliated with the GMH, including Drs. PhillipKleespies, Stephen Lancey, Justin Hill, and Gabrielle Liverant. Dr. Kleespies is arisk assessment expert affiliated with the UCC. Drs. Lancey, Hill, and Liverant arestaff psychologists on the GMH/MADC team. Interaction with multiple supervisorsallows the Fellow to work closely with faculty with different areas of expertise.The Fellow will work closely with staff psychologists in GMH/MAD and PCBH Clinicsand meet weekly for individual supervision. GMH/MAD and PCBH teams meetregularly (e.g., weekly or biweekly) to discuss clinical cases, administrative issues,and current issues in mental health care. Fellows receive both individual and groupsupervision during which clinical, career development and research issues areaddressed.Research: Fellows may collaborate with staff on any number of funded clinicalresearch projects, which will provide opportunities for involvement in professionalpresentations and preparation of empirical and review manuscripts. Fellows areencouraged to identify research activities that would expand their current skill set.Fellowship supervisors are involved in several areas of ongoing research. Dr.Liverant is the Principal Investigator for a recently-completed VA-funded CareerDevelopment Award examining the role of emotional reactivity and regulationdeficits in unipolar depression. In addition, she served as a Co-Investigator on aVA Center Grant Project (PI: Gary Kaplan) exploring the relationship betweenemotional reactivity impairment, reward-based learning dysfunction, and smokingbehavior among individuals diagnosed with schizophrenia (data available foranalysis and write up). Dr. Kamholz recently served as Co-Investigator on threefederally-funded studies, including a longitudinal study of risk and resilience factorsfor PTSD and substance use disorders in firefighter recruits (NIMH; PI: SuzyGulliver), and two laboratory assessment studies evaluating emotional and affectregulatoryresponses among veterans (depression, VA, PI: Gabrielle Liverant;schizophrenia and smoking, VA, PI: Gary Kaplan). Data collection is complete anddata are available for analysis. Dr. Vielhauer serves as the Site PI for a funded,multi-site study (VA HSR&D; PI: Rani Elwy) examining help-seeking for depressionby VA Primary Care patients.Fellowship Training Brochure | 2013 - 2014 Training Year20
