Section 3: Labour and birth<br />

3.1: Mother’s LMP: Enter the date of the woman’s last menstrual period here, or circle<br />

“unknown”.<br />

3.2: Date of birth: Record the date of the birth here, whether live or stillborn.<br />

3.3: Gestational age: Enter in weeks and days at the time of birth (live or stillbirth), using<br />

the LMP.<br />

Choose gestational age to record in this order:<br />

1. If there is a gestational age based on early ultrasound, enter this.<br />

2. If there is no gestational age based on early ultrasound, enter the estimated gestational<br />

age according to woman’s recollection of her LMP.<br />

3. If there is no gestational age estimate either based on ultrasound or the woman’s recollection,<br />

circle “unknown” (DO NOT enter gestational age based on late ultrasound or<br />

estimated by size at delivery).<br />

3.3.1: Method of determination: Circle the method by which this gestational age was calculated.<br />

Additionally, circle “sure” or “unsure” for the LMP dates, depending on the woman’s<br />

stated level of certainty. If the woman’s certainty is not stated, or if another method was<br />

used, write this in the “other, specify” box.<br />

3.4: Place of delivery. If delivery was at a facility, enter the facility’s name on this line.<br />

3.4.1: Attendant at delivery:<br />

• Circle “midwife” if delivery was attended by a trained midwife.<br />

• Circle “nurse” if delivery was attended by a nurse with midwifery skills.<br />

• Circle “doctor” if delivery was attended by a physician.<br />

• If the delivery was attended but none of the provided options fit, write in the type of<br />

attendant in the “other” box (e.g. traditional birth attendant, community health worker,<br />

relative).<br />

• Circle “no one” if no one other than the woman was present at the delivery.<br />

• Circle “unknown” if delivery attendance is not known.<br />

3.5: Onset of labour: Circle to indicate the appropriate information:<br />

• “spontaneous” if labour began without artificial aid<br />

• “induced” if labour was brought on with the use of drugs<br />

• “c/s before onset” if a caesarean section was done before the onset of labour<br />

• “unknown” if unsure.<br />

3.6: Fetal heart sounds on admission:<br />

• If fetal heart sounds (fetal heart tones) were auscultated on admission and were not present,<br />

circle “no”.<br />

• If fetal heart sounds (fetal heart tones) were auscultated on admission and were present,<br />

circle “yes” and write what they were recorded as on admission.<br />
