1.5: Multiple pregnancies:<br />

In the “pregnancies” box, write the total number of pregnancies of at least two fetuses (e.g.<br />

twins, triplets).<br />

In the “babies” box, write the total number of fetuses or neonates who resulted from these<br />

pregnancies. Include those born alive as well as stillborn.<br />

For example, suppose that in one month a hospital delivered 10 women who had pregnancies<br />

with more than one fetus. Suppose that of these 10 women, 8 delivered live twins, 1<br />

delivered stillborn twins, and 1 delivered live triplets. In this example, the “pregnancies”<br />

box would have the number “10” and the “babies” box would have the number “21”.<br />

1.6: Born before arrival: Enter the total number of stillbirths and live births that occurred<br />

before arrival at the facility.<br />

1.7: Mode of delivery: Write in each box the total number of deliveries by cephalic vaginal<br />

delivery (CVD), vacuum-assisted, forceps-assisted, caesarean and unknown mode.<br />

1.8: Gestational age: Write in each box the total number of:<br />

• term deliveries: deliveries at gestational ages 37–42 weeks<br />

• post-term deliveries: deliveries at gestational ages > 42 weeks<br />

• extremely preterm deliveries (“ext preterm”): deliveries at gestational ages < 28 weeks<br />

• very preterm deliveries: deliveries at gestational ages 28–32 weeks<br />

• moderate-to-late preterm deliveries (“mod preterm”): deliveries at gestational ages<br />

32–37 weeks.<br />

1.9: HIV status: Record the numbers of HIV-negative mothers, HIV-positive mothers, and<br />

mothers of unknown HIV status served by the facility in the past month.<br />

1.10: Syphilis serology: Record numbers of syphilis-negative mothers, syphilis-positive<br />

mothers, and mothers of unknown syphilis status served by the facility in the past month.<br />

1.11: Maternal age: Record the numbers of mothers served by the facility in the past month<br />

within each of the age groups shown on the form, as well as number of mothers for whom<br />

age group was unknown.<br />

Section 2: Cause of death<br />

2.1a Cause of death – antepartum stillbirths: Tally the number of causes of antepartum<br />

stillbirths in each of the listed categories in the past month at this facility. If M1–M5 designations<br />

were used, total each of those. If M1–M5 designations were not used, enter all in<br />

the “other” column provided. Tally any unknowns in the “unknown” column.<br />

If a facility has stillbirths for which antepartum versus intrapartum status is unknown,<br />

record these separately in the empty space to the right of the antepartum deaths list, along<br />

the same rows.<br />
