in the high school nurse’s suite.
All athletes must have a complete
physical prior to starting the fall
sports season. Previous
physicals. Private physicals
conducted prior to June 1, 2012,
will not be accepted for the fall
season.
Reservations for a physical can
be made by contacting the Athletic

Forms are available at the West
Allegheny High School Main
Office and Athletic Office and

Forms” section. Athletes must
complete the form prior to the
physical. The physicals will be
performed by the school
physician and are free of charge.
