Requirement:<br />
• The Facilities Regulati<strong>on</strong>s, Secti<strong>on</strong> 48 relating to <strong>Well</strong> c<strong>on</strong>trol equipment requires that<br />
well c<strong>on</strong>trol equipment shall be designed and shall be capable <str<strong>on</strong>g>of</str<strong>on</strong>g> being activated so as<br />
to provide for barrier integrity as well as well c<strong>on</strong>trol. (Ref. the Facilities Regulati<strong>on</strong>s,<br />
Secti<strong>on</strong> 83, No. 2 relating to entry into force. The c<strong>on</strong>tent in the Facilities<br />
Regulati<strong>on</strong>s, Secti<strong>on</strong> 48 is the same as emerges in the previous Drilling Regulati<strong>on</strong>s <str<strong>on</strong>g>of</str<strong>on</strong>g><br />
1992.)<br />
• The Activity Regulati<strong>on</strong>s, Secti<strong>on</strong> 27 relating to Planning, which require that, in the<br />
planning <str<strong>on</strong>g>of</str<strong>on</strong>g> activities, the resp<strong>on</strong>sible party shall ensure that important c<strong>on</strong>tributors to<br />
risk are kept under c<strong>on</strong>trol, both individually and collectively.<br />
5.1.9 N<strong>on</strong>-c<strong>on</strong>formity 9. Inadequate management involvement in c<strong>on</strong>necti<strong>on</strong> with<br />
assigning priority to the peer assist review<br />
In accordance with Statoil governing documents, it is a requirement that a peer assist is to be<br />
c<strong>on</strong>ducted because this operati<strong>on</strong> is in a complex well with extensive known damage. The<br />
RESU manager is resp<strong>on</strong>sible for issuing a summ<strong>on</strong>s to the meeting.<br />
Evidence:<br />
Due to a collisi<strong>on</strong> <str<strong>on</strong>g>of</str<strong>on</strong>g> meeting times / poor attendance, the meeting was not given priority <strong>on</strong><br />
12 November 2004, but was delayed until 19 November 2004, and then cancelled. The<br />
management did not get involved, and did not ensure implementati<strong>on</strong>, guidance or<br />
prioritizati<strong>on</strong> <str<strong>on</strong>g>of</str<strong>on</strong>g> the peer assist.<br />
Requirement:<br />
• The Management Regulati<strong>on</strong>s, Secti<strong>on</strong> 11 <strong>on</strong> Manning and competence requires that<br />
the resp<strong>on</strong>sible party shall ensure adequate manning and competence in all phases <str<strong>on</strong>g>of</str<strong>on</strong>g><br />
the petroleum activities.<br />
• The Management Regulati<strong>on</strong>s, Secti<strong>on</strong> 9 relating to Planning requires that the<br />
resources necessary to carry out the planned activities shall be placed at the disposal<br />
<str<strong>on</strong>g>of</str<strong>on</strong>g> project and operati<strong>on</strong>al organizati<strong>on</strong>s.<br />
• WR 0442”Planning <str<strong>on</strong>g>of</str<strong>on</strong>g> Drilling & <strong>Well</strong> Operati<strong>on</strong>s”<br />
5.1.10 N<strong>on</strong>-c<strong>on</strong>formity 10. Inadequate approval procedures<br />
Governing documents require c<strong>on</strong>crete milest<strong>on</strong>es 26 in well planning, in part so that faults in<br />
relati<strong>on</strong> to well barriers can be discovered.<br />
Evidence:<br />
The program was approved without c<strong>on</strong>firmati<strong>on</strong> that these milest<strong>on</strong>es were achieved. The<br />
program was verified/checked, recommended and approved with faulty well barrier status.<br />
The program was approved with faults, which indicates routine signature rather than actual<br />
checking.<br />
Requirement:<br />
• The Management Regulati<strong>on</strong>s, Secti<strong>on</strong> 8 relating to basis and criteria for decisi<strong>on</strong><br />
which state that, prior to decisi<strong>on</strong>s being made, the resp<strong>on</strong>sible party shall ensure that<br />
issues relating to health, envir<strong>on</strong>ment and safety have been comprehensively and<br />
adequately c<strong>on</strong>sidered.<br />
