that pers<strong>on</strong>nel are trained in accordance with governing documents lies with the line<br />
manager/lead pr<str<strong>on</strong>g>of</str<strong>on</strong>g>essi<strong>on</strong>al engineer for the discipline.<br />
Statoil should have a system that is clear enough to show c<strong>on</strong>necti<strong>on</strong>s between requirements<br />
that are stipulated for various positi<strong>on</strong>s, and the resp<strong>on</strong>sibility that governing documents<br />

5.3 Barriers that functi<strong>on</strong>ed<br />
Emergency preparedness and evacuati<strong>on</strong>: Manning <str<strong>on</strong>g>of</str<strong>on</strong>g> the emergency resp<strong>on</strong>se center and<br />
mustering <strong>on</strong> board proceeded according to plan. The handling <str<strong>on</strong>g>of</str<strong>on</strong>g> the situati<strong>on</strong>, with the<br />
assessments that were made during the course <str<strong>on</strong>g>of</str<strong>on</strong>g> the night seem to have averted a negative<br />
development <str<strong>on</strong>g>of</str<strong>on</strong>g> the situati<strong>on</strong>.<br />
Notificati<strong>on</strong>s were made, measures were instituted to combat the problem, and<br />
normalizati<strong>on</strong>/safeguarding pers<strong>on</strong>nel took place in accordance with the established plans.<br />

discovered <strong>on</strong> the sea.<br />
Lifeboats were not used during this incident. Use <str<strong>on</strong>g>of</str<strong>on</strong>g> lifeboats was, however, c<strong>on</strong>sidered. A<br />
situati<strong>on</strong> with <str<strong>on</strong>g>gas</str<strong>on</strong>g> <str<strong>on</strong>g>blowout</str<strong>on</strong>g> <strong>on</strong> the seabed is not c<strong>on</strong>sidered to be a problem as there is the<br />
opti<strong>on</strong> <str<strong>on</strong>g>of</str<strong>on</strong>g> bringing air to the motor from inside the lifeboat (<str<strong>on</strong>g>gas</str<strong>on</strong>g> flasks).<br />
The evacuati<strong>on</strong> was carried out in two large batches (9+3 lifts). For this purpose, a Bell 214<br />
from Statfjord B and a Super Puma from Oseberg were used. The flight time to Stena D<strong>on</strong><br />
and <strong>Snorre</strong> B is 3 and 4 minutes respectively.<br />
6 ASSESSMENT AND DISCUSSION OF UNCERTAINTIES<br />
This chapter discusses areas in which c<strong>on</strong>tradictory informati<strong>on</strong> was given during interviews,<br />
or where the investigati<strong>on</strong> has been unable to uncover an exact descripti<strong>on</strong> <str<strong>on</strong>g>of</str<strong>on</strong>g> the incident, or<br />
where causal relati<strong>on</strong>s seem to be present, although this cannot be documented.<br />
In Chapters 6.1 and 6.2, the significance <str<strong>on</strong>g>of</str<strong>on</strong>g> various organizati<strong>on</strong>al changes is discussed <strong>on</strong> the<br />
basis <str<strong>on</strong>g>of</str<strong>on</strong>g> an overall assessment. The PSA believes that reorganizati<strong>on</strong>s, use <str<strong>on</strong>g>of</str<strong>on</strong>g> c<strong>on</strong>sultants and<br />
a change in the drilling c<strong>on</strong>tractor may have had an impact <strong>on</strong> inadequate prioritizati<strong>on</strong> <str<strong>on</strong>g>of</str<strong>on</strong>g><br />
resources in c<strong>on</strong>necti<strong>on</strong> with the planning. Preparati<strong>on</strong> <str<strong>on</strong>g>of</str<strong>on</strong>g> the program was not assigned high<br />
status, and was not c<strong>on</strong>sidered to be a critical activity to the necessary degree. Therefore, it<br />
was never assigned priority, in spite <str<strong>on</strong>g>of</str<strong>on</strong>g> the fact that it involved work in a well with opening to<br />
