implemented after repeated serious and less serious incidents <strong>on</strong> SNA.<br />

Requirement:<br />

• The Management Regulati<strong>on</strong>s, Secti<strong>on</strong> 19 relating to Registrati<strong>on</strong>, examinati<strong>on</strong> and<br />

investigati<strong>on</strong> <str<strong>on</strong>g>of</str<strong>on</strong>g> situati<strong>on</strong>s <str<strong>on</strong>g>of</str<strong>on</strong>g> hazard and accident require that situati<strong>on</strong>s that<br />

frequently recur shall be examined closely through investigati<strong>on</strong>.<br />

• The Activity Regulati<strong>on</strong>s, Secti<strong>on</strong> 22 relating to Procedures requires that the<br />

resp<strong>on</strong>sible party shall ensure that procedures are established and used in such a way<br />

as to fulfill their intended functi<strong>on</strong>s.<br />

• WR0015. ”Handling <str<strong>on</strong>g>of</str<strong>on</strong>g> undesirable incidents and HSE data in UPN”, Chapter 6.1.4<br />

”Incidents that are repeated”.<br />

5.1.14 N<strong>on</strong>-c<strong>on</strong>formity 14. Implementati<strong>on</strong> <str<strong>on</strong>g>of</str<strong>on</strong>g> tail pipe perforati<strong>on</strong><br />

On 21 November 2004, the 2 7/8” tail pipe was perforated in accordance with the program.<br />

The well was then opened to communicati<strong>on</strong> with the reservoir pressure <str<strong>on</strong>g>of</str<strong>on</strong>g> maximum 325 bar.<br />

After testing in December 2003, the sec<strong>on</strong>dary barriers were rated for just 94 bar.<br />

Evidence:<br />

No documentati<strong>on</strong> has been submitted to the PSA to c<strong>on</strong>firm that measures had been<br />

implemented to analyze and secure the sec<strong>on</strong>dary barrier in relati<strong>on</strong> to maximum pressure,<br />

before the tail pipe was perforated. Nor is there documentati<strong>on</strong> that the sec<strong>on</strong>dary barriers<br />

were tested for the anticipated pressure.<br />

Requirement:<br />

• The Activity Regulati<strong>on</strong>s, Secti<strong>on</strong> 76 relating to <strong>Well</strong> barriers requires that, during<br />

drilling and well activities, there shall at all times be at least two independent and<br />

tested well barriers after the surface casing is in place.<br />

• The Management Regulati<strong>on</strong>s, Secti<strong>on</strong> 2 relating to Barriers requires that "it shall be<br />

known which barriers are not functi<strong>on</strong>ing".<br />

5.1.15 N<strong>on</strong>-c<strong>on</strong>formity 15. Executing link does not stop the operati<strong>on</strong> before<br />

perforati<strong>on</strong> <str<strong>on</strong>g>of</str<strong>on</strong>g> the tail pipe<br />

When the tail pipe was perforated, the well was opened to communicati<strong>on</strong> with reservoir<br />

pressure. The executing drilling crew, with the drilling operati<strong>on</strong>s supervisor, drilling<br />

supervisor and toolpusher as pr<str<strong>on</strong>g>of</str<strong>on</strong>g>essi<strong>on</strong>al experts, did not demand a full halt in the operati<strong>on</strong>,<br />

for example for the reas<strong>on</strong> that the situati<strong>on</strong> was not sound.<br />

Evidence:<br />

The operati<strong>on</strong> was not halted due to inadequate barrier status.<br />

Requirement:<br />

• The Activity Regulati<strong>on</strong>s, Secti<strong>on</strong> 76 relating to <strong>Well</strong> barriers requires that, during<br />

drilling and well activities, there shall at all times be at least two independent and<br />

5.1.16 N<strong>on</strong>-c<strong>on</strong>formity 16. Inadequate handling <str<strong>on</strong>g>of</str<strong>on</strong>g> n<strong>on</strong>-c<strong>on</strong>formities<br />

The drilling supervisor - night inquired in an e-mail <str<strong>on</strong>g>of</str<strong>on</strong>g> 23 November 2004 to the program<br />

engineer, with a copy to the drilling operati<strong>on</strong>s supervisor, ”is it OK to pull 7 5/8” scab-liner<br />
