2 STATUS PRIOR TO THE INCIDENT<br />

2.1 History / descripti<strong>on</strong> SNA<br />

Saga Petroleum ASA was the original operator <str<strong>on</strong>g>of</str<strong>on</strong>g> SNA. Norsk Hydro AS took over the<br />

operator resp<strong>on</strong>sibility <strong>on</strong> 1 January 2000, until Statoil took over as operator <strong>on</strong> 1 January<br />

2003.<br />

SNA is an integrated living quarters, drilling and producti<strong>on</strong> facility which commenced<br />

operati<strong>on</strong>s in 1992. The facility is anchored to the seabed with tensi<strong>on</strong> legs. Two subsea<br />

producti<strong>on</strong> facilities (UPAs) are tied in to the installati<strong>on</strong>. <strong>Snorre</strong> UPA is situated <strong>on</strong> the<br />

seabed 6 km north-east <str<strong>on</strong>g>of</str<strong>on</strong>g> the facility, and Vigdis UPA is situated <strong>on</strong> the seabed 7 km southwest<br />

<str<strong>on</strong>g>of</str<strong>on</strong>g> the facility. The water depth in the area is 300-350 meters.<br />

2.2 HSE before operati<strong>on</strong>s start-up<br />

The level <str<strong>on</strong>g>of</str<strong>on</strong>g> activity <strong>on</strong> SNA has been high for several years. When the incident took place,<br />

both drilling and well interventi<strong>on</strong> were underway, as well as rigging <str<strong>on</strong>g>of</str<strong>on</strong>g> a new well<br />

interventi<strong>on</strong> derrick.<br />

At the end <str<strong>on</strong>g>of</str<strong>on</strong>g> October/beginning <str<strong>on</strong>g>of</str<strong>on</strong>g> November, SNA changed drilling c<strong>on</strong>tractors from<br />

Prosafe to Odfjell Drilling. Odfjell took over about 80 % <str<strong>on</strong>g>of</str<strong>on</strong>g> Prosafe's SNA pers<strong>on</strong>nel.<br />

Nevertheless, there were several pers<strong>on</strong>s in Odfjell's drilling crew who were new to SNA<br />

when the incident occurred. In additi<strong>on</strong>, Statoil's drilling supervisor was <strong>on</strong> his first trip to the<br />

SNA.<br />

Since September 2003, notificati<strong>on</strong> has been given <str<strong>on</strong>g>of</str<strong>on</strong>g> four serious incidents <strong>on</strong> SNA (Statoil's<br />

internal category "red"), <str<strong>on</strong>g>of</str<strong>on</strong>g> which two were well incidents and two were other types <str<strong>on</strong>g>of</str<strong>on</strong>g><br />

incidents (falling object, serious pers<strong>on</strong>al injury). In the <strong>Snorre</strong> organizati<strong>on</strong>, there is a<br />

general percepti<strong>on</strong> that SNA is am<strong>on</strong>g the Statoil facilities with the worst HSE<br />

ranking/statistics. 2<br />

Experience gained from the investigati<strong>on</strong> group's inspecti<strong>on</strong>s indicate that the working<br />

envir<strong>on</strong>ment <strong>on</strong> the facility is good.<br />

2.3 <strong>Well</strong> history and status <str<strong>on</strong>g>of</str<strong>on</strong>g> P-31A prior to the incident<br />

<strong>Well</strong> P-31 was drilled as an observati<strong>on</strong> well in 1994. The intenti<strong>on</strong> was to collect geological<br />

data to optimize the well path for the horiz<strong>on</strong>tal secti<strong>on</strong> <str<strong>on</strong>g>of</str<strong>on</strong>g> the well. The sidetrack P-31A was<br />

drilled and completed in the first half <str<strong>on</strong>g>of</str<strong>on</strong>g> 1995. The well was originally planned as a<br />

producti<strong>on</strong> well, but in early 1996 it was c<strong>on</strong>verted into an injecti<strong>on</strong> well for alternating water<br />

and <str<strong>on</strong>g>gas</str<strong>on</strong>g> injecti<strong>on</strong> (WAG). However, the well was chiefly used as a <str<strong>on</strong>g>gas</str<strong>on</strong>g> injector until it was<br />

shut in in December 2003.<br />

The following circumstances sum up the problems associated with drilling and completing the<br />

well:<br />

- During drilling <str<strong>on</strong>g>of</str<strong>on</strong>g> <strong>Well</strong> P-31 in 1994, problems arose in c<strong>on</strong>necti<strong>on</strong> with running the<br />

13 3/8” casing in the 16” well secti<strong>on</strong>. The casing had to be cut and a new well patch<br />
