As of today, iTero TM scans have been used in more than 12.6 million restorative case scans, including crown, bridge<br />

and custom implant cases*. iTero scanners have been helping to simplify the workflow of many complex cases.<br />

In this edition of the Simplifying the complex challenge, we are showcasing complex restorative cases by<br />

Dr Niraj Kinariwala (India).<br />

Full mouth rehabilitation: Implant supported upper denture<br />

Chief complaint<br />

Patient complained of loose upper teeth and difficulty in chewing<br />

Medical history<br />

The patient had a history of cardiac arrest 10 years back and has hypertension<br />

Patient was on anti-hypertensive drugs and blood thinners.<br />

Pre-treatment antibiotic coverage was given and with consent of his physician, blood thinners were skipped for 2<br />

days prior the treatment. <strong>Dental</strong> history- The lower denture was fabricated by his previous dentist 8 years ago and is<br />

implant supported fixed prosthesis<br />

Initial examinations and diagnosis<br />

Chronic periodontitis leading to periodontally compromised upper teeth owing to multifactorial reasons such as age,<br />

medication and traumatic occlusion owing to compromised teeth as a contributing factor<br />

Treatment plan<br />

Extraction of all upper periodontally compromised teeth and placement of 4<br />

implants followed by delivery of implant supported denture.<br />

Treatment workflow<br />

• [Initial consultation]<br />

Following consultation, initial scan was taken with iTero TM<br />

intraoral scanner and the posterior bite from the initial scan was<br />

used to establish tentative vertical dimension on exocad TM<br />

• [Extraction and implant placement]<br />

Extraction of upper periodontally compromised teeth from 17 to<br />

27 followed by immediate placement of 4 implants (All-on-4) with<br />

multi-unit abutments was carried out.<br />

• [Digital mock-up and Provisional]<br />

Exocad software was used with iTero scan to create the digital<br />

mock-up and it allowed patient to visualise the final outcome of<br />

the restoration. The provisional was 3D printed and delivered<br />

after 24 hours post minor adjustments, and the patient was<br />

asked to wear the denture for the next 6 months to ensure<br />

comfort before delivering the final denture.<br />

• [Post osseointegration]<br />

Post 6 months, the stability of the implant was ensured, and<br />

intraoral scan bodies were screwed on and iTero scan was taken<br />

for fabrication of implant-supported fixed denture<br />

Conclusion<br />

• Pre-treatment scan served as a communication starter during<br />

the initial consultation appointment and helped explain the<br />

patient the treatment plan.<br />

• Occlusogram tool helped explain the patient understand existing<br />

trauma from occlusion owing to upper compromised teeth acting<br />

as a contributing factor and the need to replace periodontally<br />

compromised teeth.<br />
