Medicine Cabinet - Spanish speaking and do not need to be legal in this country. Services are<br />
provided to people <strong>of</strong> all ages. No Social Security needed. Clients need to have a<br />
prescription from their doctor, intake form is completed, they send <strong>of</strong>f the<br />
information to each different pharmaceutical for the medications. The process takes<br />
about 3-4 weeks. Clients receive 3 months supply at a time and then need to reapply<br />
if services are needed again. People need to provide last 2 paycheck stubs, picture<br />
ID, prescription from Doctor. Enrollment fee $25.00, for name brand drugs $10.00<br />
and $20.00 for generic - 90 day supply.<br />

Nevada Disability Rx - Effective January 1, 2007 the state <strong>of</strong> Nevada began providing<br />
assistance with the cost <strong>of</strong> prescription medicines to qualified individuals with<br />
disabilities. Eligibility requirements are: Age 18 through 61 with verifiable<br />
disability, Nevada resident continuously for at least the last 12 months, annual<br />
income no more than $24,561 for singles and $32,742 for couples. Not Medicare<br />
eligible: no monthly premium, no deductible, co-payments <strong>of</strong> $10 for generics or<br />
$25 for preferred brands, annual coverage limit <strong>of</strong> $5,100. Medicare eligible: Help<br />
with prescription costs after reaching the Medicare Part D coverage limit. Help with<br />
monthly premiums for Medicare Part D prescription Drug Plan (if not qualified for<br />
maximum help from Medicare with that expense).<br />

with insurance coverage for prescription medicine or can assist during a Medicare<br />

drugs, $25.00 non-preferred name brand drugs if authorized as medically necessary.<br />
