2009-2010<br />
General Certification Information<br />
Issued: June 2009<br />
Please be advised of the following information regarding certifications for Team Officials & Referees in the OMHA<br />
for the 2009-2010 season:<br />
• Individuals completing Certification Clinics will be issued cards indicating Name, Program Certification, Level<br />
of Certification, and Expiry Date, on-site the day of their Clinic. There will be no Certification Number issued.<br />
• When teams are being rostered, the key element is that the participant holds the appropriate OMHA<br />
certification and that the expiry date is valid.<br />
Clinic Fees<br />
The universal date for expiring certifications is AUGUST 31 st .<br />
Effective the 2009-2010 season, expiring certifications (as of August 31 st, )<br />
will no longer be granted a grace period.<br />
Updated Development Programs Clinic Fees Schedule is included in this package.<br />
Certification Conversions / Renewals / Upgrades<br />
In an effort to serve our Centres more effectively, please review the following protocols when requesting<br />
Conversions, Renewals, and Upgrades to the various Coaching and Trainer certifications. The form to complete<br />
this process is available for download at www.omha.net<br />
CONVERSIONS FROM OTHER BRANCHES / MEMBER PARTNERS<br />
For Individuals holding valid certification from other OHF Member Partners/<strong>Hockey</strong> Canada Branches, we require<br />
the following:<br />
• Personal information (Name, Address, Phone #, Email Address, & Date of Birth)<br />
• Copy or proof of Certification<br />
• $10.00 conversion fee payable by cheque or credit card (Visa, M/C)<br />
LOST CARDS / REPLACEMENT CARDS<br />
If you have lost your OMHA Certification card, forward the following information to the OMHA Office:<br />
• Personal information (Name, address, phone number, email address, & date of birth)<br />
• $5.00 fee payable by cheque or credit card (Visa, M/C)<br />
Send all Upgrade / Renewal / Conversion / Lost Cards requests info to the attention of Sue<br />
Campbell at the OMHA Office via email to susan.campbell@omha.net or by fax to 905-780-0344.<br />
