FOURTH EDITION SDDC<strong>TEA</strong> <strong>PAM</strong>PHLET <strong>55</strong>-<strong>20</strong><br />

Installations may request additional copies. Packing lists are not required <strong>for</strong> items<br />

that do not need identification (such as empty vehicles, nested cans, or bundled shovels).<br />

However, these items must be listed on the load diagram if loaded in a truck or container.<br />

A packing list is not required <strong>for</strong> a container already having a list of contents affixed to it,<br />

such as inventories of tools or parts found in supply bulletins. The packing list will be<br />

placed on both the inside and outside of the container. The exception is <strong>for</strong> sensitive<br />

items. Sensitive items will not be listed on a packing list on the outside of the container.<br />

However, the UMO will notify the ITO that sensitive items are being shipped by<br />

completing a DD Form 1907 (Signature and Tally Record). It will accompany the GBL<br />

to create a “chain-of-custody” IAW DoD Reg 4500.9-R, Part II.<br />

NOTE: DA Form 5748-R, Shipment Unit Packing List and Load Diagram, is an<br />

authorized substitute <strong>for</strong> DD Form 1750 and FORSCOM Form 285-R.<br />

The following general guidance is provided to assist in completing this <strong>for</strong>m:<br />

1. Block 1 reflects the number of box/boxes (if used asa consolidated list).<br />

2. Block 2a is the vehicle number.<br />

3. Block 2b is the SUN.<br />

4. Block 3 (End item) contains the description of the contents of the container/box/<br />

pallet.<br />

5. Blocks (d), (e), and (f) reflect the “Packs,” “Weight,” and “Total Weight,”<br />

respectively. The Total Weight column is the total <strong>for</strong> that line only. The “Total<br />

Weight” at the bottom of the <strong>for</strong>m is the cumulative weight of all the contents.<br />

6. The blocking, bracing, packing, crating, and tiedown materials (BBPCT) are<br />

entered under the contents column and the weight added to the total.<br />

7. The description of container and bumper number entries identify the container/<br />

vehicle on which the load is carried.<br />

8. The primary or alternate UMO must certify the load by signing block 6.<br />
