42Objetivo: Id<strong>en</strong>tificar factores de riesgo asociadoscon bajo peso al nacer <strong>en</strong> <strong>el</strong> Hospital de Barranca.Material y MétodosSe realizó un estudio analítico de tipo casocontrol<strong>en</strong> los recién nacidos <strong>en</strong> <strong>el</strong> Hospital deBarranca <strong>en</strong> <strong>el</strong> periodo Junio 2009- Mayo 2010,se s<strong>el</strong>eccionaron 73 casos y 112 controles, losdatos fueron tomados d<strong>el</strong> Sistema InformáticoPerinatal y se revisaron las Historias Clínicas d<strong>el</strong>os neonatos incluidos <strong>en</strong> <strong>el</strong> estudio a través defichas prediseñadas para <strong>el</strong> estudio. Se analizaránaspectos como: infección urinaria, control pr<strong>en</strong>atal(CPN), edad de la gestante. Se id<strong>en</strong>tificaron comocasos a recién nacidos(RN) con peso m<strong>en</strong>or2500gr y como controles a recién nacidos conpeso mayor 2500 gr.Resultados: Durante <strong>el</strong> periodo de estudioocurrieron 1733 partos, 1102(63.59%) eutócicosy 631(36.41%) distócicos, 73(4.21%) fueron RNvivos con bajo peso de los cuales <strong>el</strong> 54.8% fueronde sexo fem<strong>en</strong>ino.De los factores estudiados la ITU materna y lafalta de CPN estuvieron asociados con bajo pesoal nacer con un OR de 3.74 (IC: 1.92-7.32) y3.39 (I.C:1.70-6.76) respectivam<strong>en</strong>te. Respectoa la edad materna se <strong>en</strong>contró que la edadesmaternas de 20 a 24 años y mayor de 35 añosestuvieron asociados con bajo peso al naceraunque no fue estadísticam<strong>en</strong>te significativa, conun OR de 1.31 (IC:0.88-1.94) y 2.66 (IC:0.90-8.08)respectivam<strong>en</strong>te.Conclusiones: La ITU y la falta de CPN sonfactores de riesgo para bajo peso al nacer.La edad materna no ti<strong>en</strong>e influ<strong>en</strong>cia <strong>en</strong> <strong>el</strong> BPN.Se hace necesaria la detección oportuna y <strong>el</strong>tratami<strong>en</strong>to adecuado de la ITU así como lapromoción d<strong>el</strong> CPN <strong>en</strong>tre las gestantes, paraprev<strong>en</strong>ir <strong>el</strong> bajo peso al nacerMaternal risk factors associated with lowbirth weight on hospital barranca, june 2009-may 2010ABSTRACT:Introduction: Low birth weight (LBW) is a majorpublic health problem, especially in underdev<strong>el</strong>opedcountries. WHO estimates that the global rate is17%. La gripe pandémica (H1N1) 2009 afectóespecialm<strong>en</strong>te a niños y adolesc<strong>en</strong>tes, si<strong>en</strong>do la gripepandémica una <strong>en</strong>fermedad de alta incid<strong>en</strong>cia y bajamortalidad <strong>en</strong> este grupo etario.Objetivos: Determinar las características clínicas yepidemiológicas <strong>en</strong> niños con infección por influ<strong>en</strong>zaAH1N1 <strong>en</strong> <strong>el</strong> HONADOMANI San Bartolomé de junio2009 hasta diciembre 2009.Paci<strong>en</strong>tes y Método: Se revisaron todas las historiasclínicas (30) de los paci<strong>en</strong>tes pediátricos condiagnóstico confirmado a través de resultado positivoutilizando PCR tiempoResultados: Se <strong>en</strong>contró que <strong>el</strong> grupo etáreo másfrecu<strong>en</strong>te correspondió a los m<strong>en</strong>ores de 5 años conun 66.3% (rango de 8 meses a 16 años); <strong>el</strong> 53.3%de casos fueron mujeres, <strong>el</strong> 36.7% fueron de niv<strong>el</strong>socioeconómico bajo. El 60% de los casos tuvocontacto con personas con síntomas respiratorioslos 15 días anteriores a la <strong>en</strong>fermedad; y <strong>el</strong> 11%de los casos recibieron vacuna contra la influ<strong>en</strong>zaestacional. En la Clínica se <strong>en</strong>contró que <strong>el</strong> 100%de los niños pres<strong>en</strong>taron fiebre con una media de39.5ºC y de estos 63.3% pres<strong>en</strong>taron temperaturamayor o igual a 39ºC. La tos la pres<strong>en</strong>taron <strong>el</strong> 90%de los casos, rinorrea <strong>el</strong> 86.7%, malestar g<strong>en</strong>eraly ast<strong>en</strong>ia <strong>en</strong> <strong>el</strong> 6.7% y 53.3% respectivam<strong>en</strong>te. El83.3% de los paci<strong>en</strong>tes cursaron con taquipnea ysibilancias, seguidos por tirajes y subcrepitos <strong>en</strong><strong>el</strong> 76.7% y 73.3% respectivam<strong>en</strong>te. Asimismo, seid<strong>en</strong>tifico que <strong>el</strong> 23.3% de los casos t<strong>en</strong>ían algúngrado de desnutrición. Exám<strong>en</strong>es complem<strong>en</strong>tarios:casi <strong>el</strong> 90% de los paci<strong>en</strong>tes pres<strong>en</strong>taron ProteínaC reactiva m<strong>en</strong>or de 32 mg/dl. Prevaleció <strong>el</strong> patrónradiográfico intersticial al inicio de la <strong>en</strong>fermedad <strong>en</strong><strong>el</strong> 40% de los casos, seguido por <strong>el</strong> patrón alveolar <strong>en</strong><strong>el</strong> 36.7%. Las comorbilidades que prevalecieron Asmabronquial <strong>en</strong> <strong>el</strong> 60% de los paci<strong>en</strong>tes, seguida por SOB<strong>en</strong> <strong>el</strong> 30%; además se evid<strong>en</strong>ció que <strong>el</strong> 16.7% de loscasos tuvieron anteced<strong>en</strong>tes de prematuridad y <strong>el</strong> 105anteced<strong>en</strong>tes de epilepsia. Complicaciones: las másfrecu<strong>en</strong>tes Neumonía bacteriana con un 73.35 de loscasos, seguida de 46.7% con at<strong>el</strong>ectasias durante suhospitalización, insufici<strong>en</strong>cia respiratoria <strong>en</strong> <strong>el</strong> 405 d<strong>el</strong>os casos, neumonía viral <strong>en</strong> <strong>el</strong> 30% y sepsis <strong>en</strong> <strong>el</strong>26.7% casos; <strong>el</strong> 66.7 % de los casos ameritaron apoyooxig<strong>en</strong>atorio y <strong>el</strong> 205 requirió v<strong>en</strong>tilación mecánica. El90% de los casos fueron tratados con Os<strong>el</strong>tamivir y laduración d<strong>el</strong> esquema más frecu<strong>en</strong>te fue de 5 días.El 76.7 % de los casos recibieron antibioterapia porinfección bacteriana agregada. El 23.35 de los casosrequirió manejo <strong>en</strong> la unidad de Terapia Int<strong>en</strong>siva(UTIP). 2 paci<strong>en</strong>tes (6.7%) fallecieron.Conclusiones: La infección por la gripe pandémicaInflu<strong>en</strong>za AH1N1, afecto mayorm<strong>en</strong>te a m<strong>en</strong>ores de5 años, población <strong>en</strong> las que se <strong>en</strong>contró patologíasde base ─como <strong>en</strong>fermedades crónicas─ que lespredispusieron a desarrollar formas graves de gripepandémica. Es importante recordar que <strong>el</strong> periodoposterior al de máxima actividad, la int<strong>en</strong>sidad de lapandemia disminuye por debajo de la observada <strong>en</strong> <strong>el</strong>mom<strong>en</strong>to álgido.
