tendance en faveur de l’oxaliplatine (5,7 mois versus 3,8 mois), cet objectif n’a pas été atteint, la
différence n’étant pas signicative (p=0,081). Une seconde étude (REAL 2) a comparé cisplatine à
oxaliplatine chez 964 patients atteints de cancers oesogastriques. Il s’agissait d’un essai de non infériorité
qui conclut à l’absence d’infériorité de l’oxaliplatine sur le cisplatine, les médianes de survie étant
comparables autour de 10 mois. La toxicité hématologique de grade 3-4 et les événements thromboemboliques
ont été réduits dans le bras oxaliplatine, où plus de diarrhées ont été observées. Il n’y a pas
de différence de qualité de vie. Le taux de réponse du bras EOX (épirubicine 50 mg/m 2 et oxaliplatine 60
mg/m 2 toutes les 3 semaines, capécitabine 625 mg/m 2 deux fois par jour en continu de J1 à J21) a été de
46,8% et la survie significativement supérieure à celle observée avec le protocole de référence ECF
(médiane : 11,2 mois versus 9,9 mois).
