Carcinome hépatocellulaire avancé
Deux études de phase I et 6 études de phase II ont utilisé l’oxaliplatine dans le carcinome
hépatocellulaire, en association avec des médicaments variables : topotécan, 5-FU, capécitabine,
gemcitabine, FUDR et Lipiocis. Les taux de réponse sont modestes, < 20% dans la plupart des études.
Aucune étude randomisée n’est disponible et les essais thérapeutiques doivent se poursuivre pour
déterminer la place de la chimiothérapie dans le carcinome hépatocellulaire.

Cancer de l’œsophage
Une étude de phase I a été publiée en 2002 testant une chimioradiothérapie préopératoire avec du 5-FU
en perfusion continue et de l’oxaliplatine. Les taux de réponse (81% de réponse complète histologique)
étaient très encourageants. Trois études de phase I ont été présentées depuis. Une étude randomisée
multicentrique française (L93-26) est en cours, comparant une chimioradiothérapie (avec Folfox 4) au
schéma de radiothérapie de référence du RTOG (avec 5-FU, cisplatine, dit schéma de Herskovic). L’étape
de phase II randomisée est terminée et est en cours d’analyse et une phase III devrait débuter en juin
2006. Les premiers résultats prometteurs doivent être confirmés par la phase III et, dans l’attente des
résultats, il n’y a pas lieu d’envisager une autorisation temporaire de traitement.
