Maatwerk ook buiten het UMCG
Maar maatwerk beperkt zich niet tot de muren van het
ziekenhuis. Ook na het ontslag voelt het UMCG zich
medeverantwoordelijk. Daarom werkt het ziekenhuis
al jaren samen met perifere ziekenhuizen, thuiszorg,
verpleeg- en verzorgingshuizen. Het UMCG heeft met
umcGroningen Thuis zelfs een eigen thuiszorgorganisatie
in huis. Hier werken verpleegkundigen die zijn gespecialiseerd
in het leveren van zorg aan mensen met specifieke
ziektebeelden, zoals diabetes, hart- en vaatziekten,
multiple sclerose, reuma, astma en andere longaandoeningen.
Samen met de cliënt zoekt de verpleegkundige
naar betere of nieuwe zorgoplossingen en daarin gaat
umcGroningen Thuis ver. De gespecialiseerde thuiszorgverpleegkundigen
werken vanzelfsprekend nauw samen
met de andere disciplines van het UMCG.
Samenwerking met huisartsen
In Nederland is de huisarts de spil in de zorgverlening.
Het UMCG werkt daarom intensief samen met de huisartsen
in zijn regio en organiseert regelmatig symposia
en bijscholingscursussen. Sinds 2004 is er in het UMCG
bovendien een huisartsenpraktijk gevestigd. De huisartsen
die hier werken zijn in dienst van het ziekenhuis.
Naast eerstelijns patiëntenzorg houden zij zich bezig
met wetenschappelijk onderzoek. De huisartsenpraktijk
brengt huisarts en ziekenhuis dichter bij elkaar, zodat
huisartsen en medisch specialisten hun werk nog beter
op elkaar kunnen laten aansluiten.
Customization also outside the UMCG
However, customization is not limited to the walls of the
hospital. Even after the patient is discharged the UMCG
accepts that it has an ongoing responsibility. This is why,
for many years, the hospital has worked together with
peripheral hospitals, home care providers, care homes
and nursing homes. With umcGroningen Thuis (UMCG
at home) the UMCG even has its own home care organization.
This organization employs nurses who specialize
in providing care to people with specific syndromes, such
as diabetes, cardiovascular disease, multiple sclerosis,
arthritis, asthma and other lung diseases. Together with
the client the nurse looks for improved or new care solutions
and umcGroningen Thuis goes a long way toward
this objective. Needless to say, the specialist home care
nurses work together closely with the other disciplines
of the UMCG.
Collaboration with General Practitioners
In the Netherlands the General Practitioner (GP) is the
axis in the provision of healthcare. This is why the UMCG
collaborates intensively with the GPs in its region and
regularly organizes symposiums and refresher courses.
Since 2004 the UMCG has also a GP practice. The physicians
who work here are employed by the hospital. In
addition to first-line patient care they are involved in research.
The GP practice brings the GP and the hospital
closer together, so that GPs and medical specialists are
able to coordinate their activities even better.
