De derde evolutie tot slot is de opkomst van AI en de integratie van big data in
de operatierobot. De bedoeling is dat de machines op een slimme manier gaan
‘leren’ van alle big data die wordt gehaald uit een patiënt. Een scan van een
patiënt kan bijvoorbeeld met 3D en Virtual Reality inzichten geven in de persoonlijke
anatomie van een patiënt. Op die manier kan een complexe ingreep een beter
resultaat geven. In het programma ‘Topdokters’ konden we bijvoorbeeld zien
hoe prof. dr. Karel Decaestecker van het UZ Gent (zie kaderstuk) een niertumor
moest verwijderen met de operatierobot. Hij stond daarbij onder grote tijdsdruk
(hij had slechts twintig minuten) om de tumor te verwijderen, want een nier sterft
af wanneer de slagader te lang wordt afgeklemd. Via Augmented Reality kan de
chirurg beelden maken van situaties en problemen in real-time.

Where is robotic surgery t oday?
The US market for robotic surgery has been growing at an impressive rate since inception,
with growth of 14 % and 17% year over ye ar for the previous two years for Intuitive. The
company reported 753,000 procedures in the US alone in 2018, with guidance of 15 -17%
in worldwide procedure growth expected in 2019. Judging by the maintenance of these
here is no question we are still in early stages for
robotics. Specifically when taking a look at the trailing 5 -year growth rates for these
procedures categories we can see general surgery has been growing an impressive 30 -
35% year over year (chart 2) . Se veral features of the technology can be attributed to
driving the growth we’ve seen in recent years, most of which have been described as
making surgeon’s life easier in the operating suite. Back tracking to the 1960s and 70s we
saw the rise of laparoscopi c surgery in gynecology due mainly to enhanced surgical access
to the pelvic cavity. In the late 1980s we finally saw this technology utilized in the
abdomen for general surgery procedures. Within general surgery, numbers though, the at an drive increasing was larger base t
less about ergonomics, but rather improved clinical outcomes from smaller surgical
incisions. Large open abdominal incisions are associated with increased morbidity and
mortality from surgical site infection, wound dehiscence, ventral hernia, and post -
operative pain, among others. Laparoscopic surgery has helped to mitigate many of these
post-op complications, but if it has been so successful why robotics?
De markt voor chirurgie met behulp van robotica en artificiële intelligentie en big
data is bijzonder groot. Zo heeft een studie van Bank of America Merrill Lynch
aangetoond dat er enkel in de Verenigde Staten al zowat 21 miljoen chirurgische
ingrepen gebeuren op jaarbasis. Op basis van analysewerk komt de studie tot
de conclusie dat er tot 6 miljoen procedures per jaar in de VS uiteindelijk kunnen
gebeuren met robotica. In 2018 bedraagt de penetratiegraad op dit moment
slechts 10 tot 20 procent. Meer dan 60 procent van alle operaties die wereldwijd
gebeuren en waarbij de robot zou kunnen helpen, gebeurt nog steeds aan de
hand van een klassieke ‘open’ techniek. Van de <40 procent die gebeurt met
een minimaal invasieve techniek, gebeurt naar schatting minder dan 5 procent
met de robot. Er is dus nog een enorm groeipotentieel.

From our experience speaking with surgeons and attending the SRS conference it is clear
that there are many pain points that robotics has help to address in the OR. Visualization
has of course always been a limitation in abdominal and pelvic surgery. Yes, laparoscopy
has made tremendous bounds here, but the imaging capabilities between lap and robotic
systems hardly compare today. With lap you have an excellent high-definition camera
attached to the end of the scope that projects to video screens in the OR. The problem is
