Therapeutic Goals<br />
a. Not infrequently, anxiety worsens the acute asthma attack. The increased smooth<br />
muscle tone in the bronchi is, in part, caused by the anxiety of the patient <strong>and</strong> those<br />
around him. The tendency for anxiety is also present in the chronic patient. The objective<br />
is, therefore, to help decrease the anxiety, both in the acute attack <strong>and</strong> in remission.<br />
b. The barrel chest in severe asthmatics <strong>and</strong> COPD is based on hypertonic muscles that<br />
put the chest in an inhalation position. The therapeutic goal is to relax the inhalation<br />
position of the thorax.<br />
c. The increased muscle tone in asthma can chiefly, but not exclusively, be found in <strong>and</strong><br />
around the respiratory tract. It is matched by increased activity of the nervous system.<br />
Efferent motor nerves innervate the surrounding voluntary muscles of the chest, neck,<br />
<strong>and</strong> abdomen, including the diaphragm. The smooth muscles of the bronchi are<br />
activated by the parasympathetic nervous system. Both play an important role in the<br />
pathophysiology of hypertonic musculature in asthma. The objective is to achieve a<br />
relaxation of the muscle tone of these voluntary <strong>and</strong> involuntary muscles.<br />
The general objective of a, b, <strong>and</strong> c is to achieve relaxation.<br />
d. An existing metabolic acidosis needs to be corrected.
